Summary
Rhythm disorders are severe for pacemaker/ICD patients, if they cause acute severe symptoms and make an immediate treatment necessary. Appropriate diagnostic methods are the surface ECG, provocation tests, diagnostic device counters, and the chest X-ray. Severe rhythm disorders are in pacemaker patients a bradycardia after pacemaker implantation and pacemaker tachycardias in patients with dualchamber devices. Possible causes for a bradycardia after pacemaker implantation are technical device failure, battery depletion, lead dysfunction, increase of the pacing threshold, oversensing, activation of pacing functions such as hysteresis or night rate decrease, inappropriate connections between pacing lead and device, inappropriate device programming, atrial undersensing or the pacemaker syndrome. Possible causes for pacemaker tachycardias are the pacemaker-mediated tachycardia (PMT), atrial flutter with 2 : 1-conduction, atrial fibrillation, sinus tachycardia or too sensitive sensor programming for rate-responsive pacing. Severe rhythm disorders in ICD patients are inadequate shocks, frequent appropriate shocks, ventricular tachycardias above the programmed tachycardia detection interval, ventricular tachycardia due to pacemaker stimulation and sudden cardiac death despite ICD implantation.
Zusammenfassung
Herzrhythmusstörungen sind für Schrittmacherbzw. ICD-Patienten bedrohlich, wenn sie zu erheblichen akuten Beschwerden führen und eine rasche Behandlung erfordern. Diagnostische Methoden sind das Oberflächen-EKG, Provokationstestungen, die diagnostischen Aggregatspeicher und die Röntgen-Thoraxaufnahme. Bedrohliche Rhythmussstörungen sind bei Schrittmacherpatienten die Bradykardie nach Schrittmacherimplantation sowie Schrittmachertachykardien bei Patienten mit Zweikammerschrittmachern. Mögliche Ursachen für eine Bradykardie nach Schrittmacherimplantation sind der Aggregatausfall, die Batterieschöpfung, Sondenkomplikationen, ein Reizschwellenanstieg, Oversensing, die Aktivierung von Spezialfunktionen wie der Hysterese oder Nachtabsenkung, Konnektions- und Programmierfehler, atriales Undersensing oder das Schrittmachersyndrom. Bei den Schrittmachertachykardien sind die schrittmacher-vermittelte Tachykardie (PMT=pacemaker-mediated tachycardia), Vorhofflattern mit 2:1-Überleitung, Vorhofflimmern, eine Sinustachykardie oder eine zu empfindliche Einstellung des Sensors für die Frequenzadaption voneinander abzugrenzen. Bedrohliche Rhythmusstörungen bei ICD-Patienten sind inadäquate Therapien des Defibrillators, gehäufte adäquate Entladungen, Kammertachykardien oberhalb der programmierten Detektionsfrequenz, Kammertachykardien infolge von Schrittmacherstimulation und der plötzlichen Herztod trotz ICD-Implantation.
