Zusammenfassung
Herzchirurgische Patienten, die unter Anwendung der extrakorporalen Zirkulation operiert werden, sind einem erhöhten Infektionsrisiko durch nosokomiale, gram-negative Erreger ausgesetzt. Am häufigsten ist hierbei der Respirationstrakt betroffen; ursächlich ist in der Mehrzahl der Fälle Pseudomonas aeruginosa. Im Gegensatz hierzu steht bei nicht-nosokomialen Pneumonien die Infektion durch gram-positive Erreger im Vordergrund. Wir berichten in dieser Kasuistik über eine seltene, nach Einsatz der Herz-Lungen-Maschine früh postoperativ aufgetretene, nicht-nosokomiale E. coli-Pneumonie mit fulminantem Verlauf. Diese tritt mit einer Inzidenz von 0,7% auf und ist mit einer Letalität zwischen 60 und 90% assoziiert. Manifestiert sich eine fulminante Pneumonie in der frühen postoperativen Phase, so sollte auch eine präklinisch erworbene Infektion in Betracht gezogen werden. Diese erfordert aufgrund des ungewöhnlichen Erregerspektrums eine Ausweitung der für die Abdeckung nosokomialer Keime üblichen chemotherapeutischen Therapie.
Summary
Heart surgery patients who are operated on with support of extracorporeal circulation are at high risk of gram-negative nosocomial infections. The most common infection is pseudomonas aeruginosa infection of the respiratory system. In contrast, non-nosocomial pneumonias are mostly due to gram-positive pathogens. We report the case of a fulminant course of a rare non-nosocomial E. coli pneumonia during the very early post-operative period after use of extracorporeal circulation. This occurs with an incidence of 0.7% and is associated with a mortality rate between 60 and 90%. In cases of occurrence of pneumonia with fulminant course in the very early post-operative period, community acquired infections should also be taken into consideration. Because of the uncommon bacterial spectrum, a broadening of the usual intensive care unit antibiotic treatment is required.
