Gerinnungsprobleme an der Herz-Lungen-Maschine durch grüne Bauchtücher

Zusammenfassung

Wir berichten über 6 Fälle von Gerinnungsproblemen während Herzoperationen. Dreimal traten die Gerinnsel nur im Hartschalenreservoir auf, zweimal zusätzlich auch im venösen Beutel und einmal kam es zum Ausgussthrombus mit Verstopfung des Oxygenatoreingangs. Bei diesem Patienten schaltete die arterielle Pumpe ab. Keiner der Patienten, bei denen es zu Gerinnungsproblemen kam, erlitt durch die Gerinnselbildung einen Schaden. Alle machten einen komplikationslosen postoperativen Verlauf durch. Ursache der Gerinnselbildung waren grüne Bauchtücher, die durch fehlerhafte Fabrikation die Heparinwirkung inaktivierten. Nach Verbannung der grünen Bauchtücher als Unterstützungshilfe des Herzens während der Koronarchirurgie traten keine Gerinnungsprobleme mehr auf. Auch vertraute Gegenstände können durch Fehlfabrikation plötzlich Ursache von Problemen werden. Dies bürdet jedem Anwender ein noch größere Maß an Verantwortung auf.

Summary

Six patients with clotting problems during open heart operations are presented: 3 times the clots were found in the hard shell cardiotomy reservoir and 2 times additionally in the venous collapsable bags. In one patient the clot occluded the entrance of the oxygenator. As a consequence the arterial pump was switched off. None of the patients were harmed by the clots; all had an uneventfull postoperative recovery. The clotting problems during coronary surgery were caused by green towels. Due to incorrect production of the towels the Heparin was inactivated, nicely demonstrated by the use of the Heparinase test cartridge. After prohibiting the use of the green towels no more clots were seen. Commonly used items can be the cause of problems due to incorrect production. Every customer should be aware of this problem.
