Kombinierte Eingriffe bei Marfan-Syndrom

Zusammenfassung

Wir berichten über akute und elektive Eingriffe bei Marfan-Syndrom (MFS) mit familiärer Disposition. Nachdem der 42-jährigen Patientin bei positiver Familienanamnese notfallmäßig bei Aortenruptur ein Composite-Graft nach Cabrol implantiert wurde, stellte sich die 19-jährige Tochter der Patientin bei klassischem MFS mit ausgeprägter Thoraxwanddeformität in unserer Marfansprechstunde vor. Dabei wurde zusätzlich echokardiographisch eine Mitralklappeninsuffizienz III. Grades bei Floppy Valve und Chordaabriss am posterioren Segel diagnostiziert. Es erfolgte bei ihr elektiv neben einer Mitralklappenrekonstruktion nach Gerbode eine Korrektur der Thoraxwanddeformität nach Robicsek.

Summary

We report about acute and elective surgery intervention on Marfan syndrome (MFS) with family disposition. After the emergency operation for aortic rupture with implantation of a composite graft according to the Cabrol procedure on a 42-year-old female patient with positive family history, her 19-year-old daughter presented with the typical manifestations of MFS with thoracic deformities during our Marfan practice hours. The echocardiogram showed, in addition, a high mitral valve incompetence with a floppy valve and chorda rupture. An elective operation with a mitral valve repair according to the Gerbode technique was performed and the thoracic deformities, applying the modified technique described by Robicsek, were corrected.
