Verletzung der Aorta durch einen pneumatischen Klammerapparat – Eine Kasuistik

Zusammenfassung

Wir berichten über einen 27-jährigen Mann, der sich beim Arbeiten zu Hause mit einem pneumatischen Klammerapparat eine Verletzung der thorakalen Aorta zugezogen hat. Nach der Diagnostik in einem auswärtigen Krankenhaus kam der Patient mit Verdacht auf Verletzung der Aorta ascendens in instabilem Zustand in unsere Klinik. Über eine sofort durchgeführte mediane Sternotomie konnte die Verletzung an der Aorta problemlos übernäht werden. Der Patient konnte nach 5 Tagen nach Hause entlassen werden.

Summary

We report the case of a 27 year old mean with an accidental staple-gun shot injury of the chest. The staple was removed by the patient himself soon after the accident. In a nearby institution an echocardiography and computed tomography showed minimal pericardial effusion. Arriving at our clinic, the blood pressure was 80/40 mmHg, heart rate was 115 bpm. Echocardiography showed pericardial effusion with signs of cardiac tamponade. Due to the instable vital signs we performed a median sternotomy immediately. The staple that had been removed had penetrated the chest in the second intercostal space on the right side, close to the sternal bone and the ascending aorta 2 cm proximal to the aortic valve. With one suture the injury of the aorta was closed. The patient was discharged from hospital 5 days later.
