Endoluminale Therapie thorakaler Aortenaneurysmen bei Patienten nach Herztransplantation

Zusammenfassung

Wir berichten über zwei Patienten mit einem Aneurysma der deszendierenden thorakalen Aorta nach Herztransplantation vor über zehn Jahren. Eine offene chirurgische Rekonstruktion war auf Grund der schlechten kardialen Situation der Patienten nicht möglich. Die Aneurysmaausschaltung erfolgte jeweils durch transfemorale Implantation eines mit Dacron ummantelten, selbstexpandierenden Nitinolstents (Talent, World Medical Systems, Sunrise, FL). Der Eingriff erfolgte beim ersten Patienten elektiv, beim zweiten Patienten im Stadium der gedeckten Ruptur. Die endoluminale Therapie thorakaler Aortenaneurysmen stellt unter gewissen Voraussetzungen die einzige Behandlungsmöglichkeit bei nicht operablen Patienten dar. Es werden die spezifischen Probleme der endovaskulären Aneurysmakorrektur beim immunsupprimierten, herztransplantierten Patienten diskutiert.

Summary

We report about two patients with an aneurysm of the descending thoracic aorta after heart transplantation about ten years ago. – Both patients were not fit for open surgical reconstruction because of their poor cardial condition. Treatment was done by transfemoral implantation of a Dacroncovered self expanding Nitinol stentgraft (Talent, World Medical Systems, Sunrise, FL). One patient was stented electiv; the second patient was treated in phase of covered rupture. Endoluminal treatment of thoracic aneurysm represents the only possibility for patients who are not fit for open heart surgery. The special problems of endovascular reconstruction of aneurysm on immunsuppressed, heart trasnplanted patients are discussed.
