Zusammenfassung
Die Myasthenia gravis (MG) ist eine Autoimmunerkrankung, bei der polyklonale IgG-Autoantikörper mit den postsynaptischen Acetylcholin-Rezeptoren der neuromuskulären Endplatte reagieren. Die Bildung dieser Autoantikörper nimmt nach gängigen pathogenetischen Konzepten ihren Ausgang im Thymus, so daß eine kausale, Therapie in der möglichst frühzeitigen Thymektomie besteht. Im weiteren bildet die pharmakologische Immunsuppression durch Glukokortikoide und Azathioprin, in resistenten Fällen auch durch Cyclosporin A, Methotrexat oder Cyclophosphamid eine Hauptsäule der MG-Therapie. Für die Akutintervention bei einer myasthenen Krise scheint die intravenöse Gabe von Immunglobulin G (IgG) ähnlich wirksam zu sein wie die bisher applizierte Plasmapherese oder Immunabsorption. Die vorliegende Übersicht faßt Aktuelles zu Wirkmechanismen, unerwünschten Effekten und zur praktischen Handhabung der in der Behandlung der MG eingesetzten Immunpharmaka zusammen.
Summary
Myasthenia gravis (MG) is an autoimmune disease. It is mediated by polyclonal immunoglobulin G (IgG) autoantibodies, directed at the postsynaptic acetylcholine receptors of the neuromuscular endplate. The initial steps leading to the production of these autoantibodies are to supposed take place in the thymus. Consequently, early thymectomy serves as a causal therapeutic intervention. Further measures consist in the application of immunosuppressive drugs like glucocorticoids and azathioprine. In case of unsatisfactory responses, other compounds, e.g. cyclosporin A, methotrexate or cyclophosphamide, may be used. To treat myasthenic crisis, intravenous immunoglobulin treatment seems to be similarly effective as plasmapheresis or immunoabsorption. The present review summarises current concepts regarding the mode of action, the side effects and the practical use of immunosuppressants in treating MG.
