Zusammenfassung

Hintergrund

Lösungen zur Induktion des kardioplegischen Herzstillstandes existieren seit mehreren Dekaden. Seit Anbeginn der Herzchirurgie haben sich die Patientenpopulation und mit ihr das Konzept der Kardioprotektion verändert.

Fragestellung

Darstellung der modernen Konzepte der Kardioplegie unter Berücksichtigung der sich verändernden Patientenpopulation.

Material und Methoden

Auswertung vergangener sowie derzeitiger Entwicklungen kardioplegischer Lösungen.

Ergebnisse

Das Prinzip der Induktion des kardioplegischen Herzstillstandes existiert seit über 50 Jahren. Bislang dominierten überwiegend hyperkalämische, depolarisierende Kardioplegien das Feld. Basierend auf verschiedenen Wirkungsmechanismen existieren weiterhin polarisierende Kardioplegien, welche dem Operateur eine lange unterbrechungsfreie chirurgische Arbeit ermöglichen. Diese erweisen sich insbesondere bei immer häufiger auftretenden langen, komplexen Fällen als vorteilhaft.

Diskussion

Die modernen Konzepte der Kardioplegie richten sich zunehmend nach der Maßgabe, bei komplexen, lange andauernden Prozeduren eine reapplikationsfreie chirurgische Arbeit zu ermöglichen.

Abstract

Background

Solutions for the induction of cardioplegic cardiac arrest have existed for decades. Since the inception of cardiac surgery, the patient population and the concepts for cardioprotection have changed.

Objective

Presentation of modern concepts for cardioplegia considering the changing patient population.

Material and methods

Description and analysis of past and current developments of cardioplegic solutions based on the literature.

Results

The principles of the induction of cardioplegic cardiac arrest have existed for over 50 years. Hyperkalemic depolarizing solutions have dominated the field so far. Polarizing cardioplegic solutions also exist, allowing the surgeon to perform a long procedure without interruptions.

Conclusion

Modern concepts of cardioplegia predominantly focus on enabling long, complex surgical procedures without repetitive administrations.
