Zusammenfassung
Der Pneumothorax (PTX) ist ein häufiges Krankheitsbild. Die häufigste Ursache für einen PTX ist eine Verletzung der Lunge oder des Pleuraraums im Rahmen einer medizinischen Intervention (iatrogener PTX). Spontan auftretende PTX-Ereignisse werden in primäre und sekundäre Formen differenziert. Diese Differenzierung ist maßgeblich für die Festlegung der weiteren erforderlichen Diagnostik und Therapie. Bei Patienten ohne klinische PTX-Zeichen reicht eine konservative Behandlung aus. Große PTX mit einem kollabierten Lungenvolumen von mehr als 20 % müssen behandelt werden. Ziele der PTX-Therapie sind die Wiederausdehnung der Lunge und die Vorbeugung von PTX-Rezidiven. Die Anlage von Thoraxdrainagen ist das zentrale Element der PTX-Therapie. Eine 2018 verabschiedete nationale S3-Leitlinie gibt klare Behandlungsempfehlungen zur Therapie des PTX. In diesem Übersichtsbeitrag wird die vorhandene Evidenz der PTX-Therapie zusammengefasst.
Abstract
Pneumothorax (PTX) represents a frequently occurring condition. The most common cause of PTX is injury to the lungs or the pleural cavity due to a medical intervention (iatrogenic PTX). Spontaneous PTX events are differentiated into primary and secondary PTX. This differentiation is relevant for the decision on further diagnostic and therapeutic steps. In patients without clinical signs of PTX conservative treatment is adequate. Large PTX with a collapsed lung volume of more than 20% has to be treated. The aims of all forms of PTX treatment are re-expansion of the collapsed lung and prevention of recurrence. The placement of chest drains is essential in the treatment of PTX. The national S3-guidelines adopted in 2018 provide clear recommendations for the treatment of PTX in Germany. This review summarizes the available evidence for the treatment of PTX.
