Herzentnahme zur Transplantation

Zusammenfassung

Die Herztransplantation ist perspektivisch immer noch der Goldstandard in der Behandlung der therapierefraktären terminalen Herzinsuffizienz. Die Beurteilung der Transplantationsfähigkeit eines Spenderherzens erfolgt primär aufgrund übermittelter medizinischer Spenderdaten und abschließend bei der Organentnahme. Die detaillierte und strukturierte Organevaluation vor Ort sollte durch einen erfahrenen Explantationschirurgen erfolgen, der sich als Mitglied eines gesamten Teams während der Multiorganspende versteht. Bei der Evaluation müssen neben makroskopischen Aspekten auch die detaillierte Erfassung möglicher pathologischer Auffälligkeiten bis hin zu invasiv abgeleiteten Füllungsdrücken des Spenderherzens erfolgen. Eine engmaschige Rücksprache des Entnehmenden mit dem implantierenden Zentrum ist rein aus logistischen Gründen sinnvoll und hilft zudem die Qualität des Organs mit den individuellen Aspekten der geplanten Empfängeroperation abzugleichen. Im Folgenden wird ein möglicher Verfahrensweg der Entnahme eines Spenderherzens zur Transplantation aufgezeigt.

Abstract

Heart transplantation remains the gold standard for the treatment of refractory terminal heart failure. Assessment of the transplantability of a donor heart is primarily based on medical data and finally defined at the time of harvest. The detailed and structured evaluation of the organ on site should be performed by an experienced explantation surgeon, who is a dedicated team member during the multiorgan donation procedure. The evaluation should include the detailed analyses of possible macroscopic pathological findings as well as invasive preload measurements of the donor heart. Close correspondence of the explantation surgeon with the implantation center is not only mandatory from logistic aspects but may further help to fit the organ quality with the requirements of the planned recipient operation. This article presents a possible strategic approach for the procurement of a donor heart for transplantation.
