Zusammenfassung

Die Patientin (Gravida 4, Para 2), stellte sich bei erhöhtem mütterlichem Alter und Placenta praevia marginalis in Schwangerschaftswoche 38+3 zur geplanten Entbindung vor. Nach Fruchtwasserabgang wurde sie somnolent, kardiopulmonal instabil und schließlich reanimationspflichtig. Bei Verdacht auf eine Fruchtwasserembolie erfolgten unter laufender kardiopulmonaler Reanimation die notfallmäßige Sectio caesarea und schließlich die perkutane Implantation eines „Extracorporeal-life-support“(ECLS)-Systems durch das mobile ECLS-Team der herzchirurgischen Klinik. Es konnte ein gesunder Säugling entbunden werden. Nach komplexem Intensivmedizin- und Rehabilitationsverlauf konnte die Patientin mit Kind in zufriedenstellendem Allgemeinzustand entlassen werden und in ihr familiäres Umfeld zurückkehren.

Abstract

A 43-year-old patient (gravida 4, para 2) presented for planned delivery because of increased maternal age and marginal placenta praevia at 38 + 3 weeks of gestation. Shortly after rupture of the amniotic fluid sac the patient became somnolent, rapidly followed by hemodynamic instability and need of cardiopulmonary resuscitation. Amniotic fluid embolism was suspected and an emergency caesarean section as well as percutaneous implantation of an extracorporeal life support system (ECLS) by the mobile ECLS team of the department for cardiac surgery were performed under ongoing cardiopulmonary resuscitation. A healthy neonate was delivered. After a complex recovery on the intensive care unit and in a rehabilitation clinic, the patient was discharged in an acceptable general condition and could return to her familial surroundings with her child.
