Zusammenfassung
Das Perikard erfüllt wichtige Aufgaben für die Herzfunktion; Veränderungen im und am Perikard können sich auf die Hämodynamik des Patienten auswirken. Am Ende der meisten Herzoperationen kann das eröffnete Perikard zwar wieder verschlossen werden, welche Auswirkungen aber die Öffnung auf die Hämodynamik hat, ist nicht zweifelsfrei bekannt. Diesbezügliche Erkenntnisse werden in der vorliegenden Arbeit dargelegt. Auch für chirurgische Techniken sind Komponenten des Perikards von großem Interesse, wie z. B. die perikardiale Umschlagfalte oder die nichtkoronare Kollateralversorgung des Myokards. Rechtsherzversagen nach Implantation eines Linksherzunterstützungssystems („left ventricular assist device“, LVAD) oder Pericarditis constrictiva stellen Komplikationen dar, in deren chirurgische Therapie das Perikard involviert ist. Das Perikard ist somit für die Herzchirurgie eine interessante Struktur, auf deren Funktionen, Anatomie, Physiologie, Embryologie und klinische Anwendung in diesem Beitrag eingegangen wird.
Abstract
The pericardium fulfills important functions for the heart and alterations in and on the pericardium can have consequences for the hemodynamics of the patient. At the end of most cardiac operations the pericardium can be closed again but the influences of an open pericardium on hemodynamic parameters have not yet been completely elucidated. Currently available data on this topic are presented in this manuscript. The pericardium is also of great interest for surgical techniques, such as the anatomical features of the pericardial fold and non-coronary collateral supply to the myocardium. Right heart failure after left ventricular assist device (LVAD) implantation or constrictive pericarditis are diseases necessitating surgical treatment involving the pericardium. Thus the pericardium is a relevant structure for cardiac surgery and in this article the function, anatomy, physiology and embryology of the pericardium as well as clinical applications are described.
