Arterielle „Switch“-Operation

Zusammenfassung

Die arterielle Switch-Operation ist heute die chirurgische Therapiemethode der Wahl für Patienten mit einer Transposition der großen Gefäße. Das Korrekturrisiko beträgt in geübten Händen unter 5 % und hängt im Wesentlichen von einem gelungenen Koronartransfer ab. Die Korrekturoperation bedeutet die anatomische Korrektur des Vitiums mit Anschluss des linken Systemventrikels an die Aorta ascendens unter Verschluss aller begleitenden Defekte und den Anschluss des rechten Ventrikels an die A. pulmonalis. Um die Transpositionsstellung der großen Gefäße auszugleichen, wurde das Lecompte-Manöver in die chirurgische Therapie eingeführt, mit dem die Pulmonalarterienbifurkation vor die Neoaorta gesetzt wird.

Abstract

The arterial switch operation has become the surgical treatment of choice for patients with transposition of the great vessels. The risk of correction in experienced hands is below 5 % and depends mainly on a successful coronary transfer. Corrective surgery means an anatomical repair of the congenital malformation with connection of the left systemic ventricle to the ascending aorta, repair of additional defects and connection of the right ventricle to the pulmonary artery. The Lecompte maneuver was introduced into surgical therapy in order to facilitate surgical repair by bringing the pulmonary bifurcation in front of the neoaorta.
