Zusammenfassung
Hintergrund
Die Elektrostimulation autonomer Nerven stellt sich als ein effektives, lokales Behandlungsverfahren mit großem therapeutischen Nutzen bei gleichzeitig geringen systemischen Nebenwirkungen dar.
Ziel der Arbeit
Dieser Beitrag befasst sich mit bisherigen Ergebnissen anatomischer und physiologischer Untersuchungen am autonomen Herz- sowie Lungennervensystem und den Möglichkeiten, dieses Nervensystem zu therapeutischen Zwecken zu nutzen.
Material und Methoden
Es wurde in den einschlägigen medizinischen Literaturdatenbanken nach autonomer Neurostimulation an Herz und Lungen gesucht. Die bisherigen Ergebnisse dieser Forschung wurden im vorliegenden Beitrag zusammengefasst.
Ergebnisse
Die Untersuchungen des autonomen Nervensystems des Herzens haben gezeigt, dass die elektrische Neurostimulation sowohl sympathisch als auch parasympathisch zu therapeutischen Zwecken genutzt werden kann. Im Gebiet der Kardiologie geht die Neurostimulation mit vielversprechenden, innovativen Therapieoptionen, beispielsweise zur ventrikulären Frequenzsenkung bei Vorhofflimmern, einher. Untersuchungen am autonomen Lungennervensystem beschränken sich auf den parasympathischen Einfluss auf die Bronchomotorik. Die elektrische Neurostimulation zu therapeutischen Zwecken am Lungennervensystem wurde bislang nicht hinreichend untersucht.
Schlussfolgerung
Die vielversprechenden Ergebnisse der therapeutischen Neurostimulation am Herzen lassen das gleiche funktionelle Potenzial für die Lungen annehmen. Daher sollten weiterreichende Untersuchungen an den Lungen erfolgen.
Abstract
Background
Electrical stimulation of the autonomic nervous system represents an effective local therapy with maximum benefits while producing minor or no systemic side effects.
Aim
This article addresses the existing anatomical and physiological investigation results on the autonomic nervous system of the heart and lungs and the possibilities to use this nervous system for therapeutic purposes.
Material and methods
Relevant literature databases were searched for preclinical and clinical investigations on the autonomic nervous system of the heart and lungs. The existing data were summarized and reviewed.
Results
The investigations on the autonomic nervous system of the heart showed possible therapeutic approaches by modulation of both the sympathetic and parasympathetic nervous system. In cardiology, neurostimulation has been proposed as a promising innovative therapeutic option to limit the ventricular frequency in patients suffering from atrial fibrillation.
Conclusion
The promising data on therapeutic autonomic neurostimulation of the heart give rise to the idea that the same functional potential could be assumed for the lungs. Therefore, further investigations should be performed on the autonomic nervous system of the lungs to evaluate the chances of putting electrical neurostimulation into therapeutic use.
