Zusammenfassung
Hintergrund
Die kathetergestützte Aortenklappenimplantation („transcatheter aortic valve implantation“, TAVI) ist ein innovatives Verfahren für Patienten mit schwerer symptomatischer Aortenklappenstenose. Ungeachtet des fehlenden Nutzenbelegs verglichen zum Goldstandard der offenen Operation und offener Fragen zu Sicherheit und Langzeitresultaten ist eine kontinuierliche massive Fallzahlausweitung in Deutschland zu beobachten.
Methoden
In einer systematischen Recherche wurden Publikationen identifiziert, aus denen Anforderungskriterien hinsichtlich Indikationsstellung, Struktur- und Prozessqualität abzuleiten waren.
Ergebnisse
Es liegen keine Studien vor, aus denen wissenschaftlich belegte Anforderungen zu Indikationsstellung, Struktur- und Prozessqualität abgeleitet werden können. Zwei randomisierte Studien liefern lediglich Hinweise auf Kriterien für die Patientenauswahl. Interdisziplinäre Positionspapiere der führenden wissenschaftlichen Fachgesellschaften in Europa und den USA geben konsentierte Empfehlungen. Als wesentliche Anforderungen werden genannt: Einsatz der TAVI nur bei Patienten mit faktischer Inoperabilität oder sehr hohem Operationsrisiko, Indikationsstellung und Durchführung durch ein interdisziplinäres Team mit mindestens einem Herzchirurgen und einem Kardiologen, Möglichkeit zur sofortigen offenen Herzoperation. Es besteht weitgehender Konsens, dass TAVI nur in Zentren mit einer Fachabteilung für Herzchirurgie eingesetzt werden sollte. Die Teilnahme an einem Register hoher methodischer Qualität wird empfohlen.
Schlussfolgerung
TAVI sollte derzeit nur bei Patienten mit schwerer symptomatischer Aortenklappenstenose und Kontraindikationen gegen eine offene Operation oder sehr hohem Operationsrisiko durch interdisziplinäre Teams in Zentren mit einer Fachabteilung für Herzchirurgie durchgeführt werden.
Abstract
Background
Transcatheter aortic valve implantation (TAVI) is a new treatment option for patients with severe symptomatic aortic stenosis. Despite a lack of scientific evidence for a benefit of the procedure compared with open surgery as the gold standard and questions about safety and long-term results, a continuous and massive increase of its application in Germany can be observed.
Methods
In a systematic search, publications suitable for deducing criteria for indication, structural and process standards were identified.
Results
No appropriate studies exist to define scientifically sound criteria for indication, structural and process standards for TAVI. Two randomized controlled trials give hints for potential patient selection criteria. Several interdisciplinary position papers of the most relevant scientific societies in Europe and North America provide recommendations for indication criteria and minimum structural and process requirements. TAVI should only be used in patients with contraindications for open surgery or maximum perioperative risk. Interdisciplinary heart teams comprising at least a cardiac surgeon and a cardiologist are mandatory for patient selection and performance of TAVI. Structural conditions to perform immediate open heart surgery are mandatory. Most recommendations require performing TAVI only in hospitals with a cardiac surgery unit. Participation in a registry is recommended.
Conclusion
Currently, TAVI should be restricted to patients with severe symptomatic aortic valve stenosis and contraindications against open heart surgery or maximum perioperative risk. It should be performed by interdisciplinary heart teams in hospitals with a cardiac surgery unit.
