Zusammenfassung

Eine gesunde 33-jährige Frau stellt sich in der 40. Schwangerschaftswoche mit einer unerwartet rasch eintretenden Geburt in einem Krankenhaus der Regelversorgung vor. Unmittelbar nach der Spontangeburt kommt es zu einer schweren atonen Uterusblutung, die mit einem Prostaglandin (Sulproston) gestoppt werden soll, sowie zu einem akuten und lebensbedrohlichen myokardialen Pumpversagen. Die notfallmäßige Anlage einer intraaortalen Ballonpumpe durch ein herzchirurgisches Zentrum durchbricht den Circulus vitiosus; die Patientin kann sicher in das Zentrum der Maximalversorgung verlegt werden. Im weiteren Verlauf kommt es zu einer vollständigen Genesung der Patientin.

Abstract

A healthy 33-year-old woman at 40 weeks gestation was admitted to a rural hospital and gave birth to a healthy child. The course after delivery was complicated by atonic uterine bleeding. An attempt to stop the bleeding with a prostaglandin (sulprostone) led to acute cardiac failure. Emergency implantation of an intraaortic counterpulsation pump performed by a team consisting of a cardiac surgeon and a perfusionist allowed circulatory stabilization and concomitant transfer to a cardiac center. The further course was uneventful, and the patient recovered completely.
