Zusammenfassung
Dieser letzte von insgesamt drei Übersichtsartikeln beabsichtigt, herzchirurgischen Facharztkandidaten die wichtigsten Fakten über Indikation und Behandlung seltener komplexer angeborener Herzfehler zu vermitteln. Verschiedene Verfahren der Palliation werden am Beispiel verschiedener Formen der Trikuspidalklappenatresie und Möglichkeiten der komplexen Korrektur am Beispiel des unterbrochenen Aortenbogens mit Ventrikelseptumdefekt in Abhängigkeit der Ausprägung der begleitenden Subaortenstenose erläutert. Die richtige Wahl des Verfahrens ist entscheidend für das Ergebnis der operativen Rekonstruktion und konsekutiv für die Prognose des Patienten.
Abstract
This final of three review articles is intended to provide the most important facts about indication and treatment of complex congenital heart defects for cardiac surgeons preparing for their board exams. Various methods of palliation are described on the basis of treatment of different types of tricuspid atresia. Possibilities for complex correction are also depicted using the example of the interrupted aortic arch and ventricular septal defect depending on the degree of the concomitant subaortic stenosis. Selecting the correct procedure is critical for outcome of surgical reconstruction and prognosis of the individual patient.
