Zusammenfassung
Schwierige Fälle werden heute in größeren Krankenhäusern von behandelnden Ärzten zusammen mit Fachspezialisten und Pflegefachleuten diskutiert, um optimale Ergebnisse zu erzielen. Das hier vorgestellte, seit über zehn Jahren erprobte und am Universitätsspital Basel entwickelte Programm erlaubt solche Konsultationen auch für kleine und abgelegene medizinische oder chirurgische Einrichtungen über beliebige Distanzen.
Der behandelnde Nichtexperte lädt die Dokumente des Patienten mit den nötigen Bildern (z. B. EKG) und seinen Fragen über das Internet (direkt über Internet oder indirekt über eine standardisierte E-Mail) auf einen zentralen Server. Experten werden automatisch um ihre Meinungen gebeten, welche von allen Mitgliedern der Gruppe eingesehen werden können.
Zusätzlich können Falldaten auf einfach zu erstellenden Formularen nach einheitlichen Kriterien gespeichert und später epidemiologisch aufgearbeitet werden.
Dasselbe Telemedizinsystem dient auch als virtuelle Universität, innerhalb derer Vorlesungen über ein Telekonferenzsystem gehalten und als Zusammenfassungen heruntergeladen werden können.
Abstract
Today in larger hospitals, treating physicians discuss difficult cases together with medical specialists in order to obtain optimal results. The telemedical system, developed at the University Hospital Basel and tested for over 10 years, offers this type of consultation to small and remote medical and surgical institutions regardless of distances.
The treating “non-expert” loads the case documentation with the necessary pictures (e.g., ECG) and his/her questions over the internet (directly via internet or indirectly via a standardized e-mail) on a central server. Experts are asked automatically for their opinions, which are then available simultaneously to all members of the group.
In addition, case data can be stored on easily generated forms in a uniform manner for epidemiological evaluation at a later time point.
The same telemedical system also serves as a “virtual university”, where lectures are stored and can take place in real time or/and can be printed out as summaries.
