Aneurysma des linksventrikulären Ausflusstrakts nach Aortenklappenprothesenendokarditis

Zusammenfassung

Nach einem Aortenklappenersatz trat bei einem 68 Jahre alten Patienten eine Prothesenendokarditis auf. Beim Klappenprothesenwechsel fand sich eine endokarditische Höhle, die lediglich debridiert wurde. Ausgehend von dieser Höhle entwickelte sich in der Folgezeit ein Aneurysma des linksventrikulären Ausflusstrakts von etwa 7 cm Durchmesser, das sich bis unter die Pulmonalarterie erstreckte und diese komprimierte. Bei der Reoperation rekonstruierten wir den linksventrikulären Ausflusstrakt durch einen Patch, wodurch gleichzeitig das Aneurysma exkludiert werden konnte. Der weitere Verlauf war unauffällig.

Abstract

After aortic valve replacement, a 68 year-old male showed signs of an infective prosthesis endocarditis. While replacing the infective prosthesis, an endocarditis induced cavity, which was cleaned and left open, was identified. From this structure, an aneurysm of the left ventricular outflow with an diameter of about 7 cm developed within 5 months reaching until below the pulmonary artery and compressing it. During the second reoperation the left ventricular outflow tract was reconstructed by a patch, excluding the aneurysm. The follow-up has been uneventful so far.
