Zusammenfassung
Heutzutage gilt die Herztransplantation (HTX) als Goldstandard der Therapie der chronischen und terminalen Herzinsuffizienz. Das akute Transplantatversagen (ATV) nach HTX ist eine dramatische Komplikation, die mit einer hohen Morbidität und Mortalität einhergeht. Als Risikofaktoren konnten bisher eine verlängerte Ischämiezeit, eine schlechte Organqualität und eine akuten Abstoßungsreaktion identifiziert werden. Folge des ATV ist nicht selten ein Multiorganversagen (MOV). Als therapeutische Optionen stehen primär konservative Maßnahmen, wie die Behandlung mit Katecholaminen und Phosphodiesterasehemmern zur Verfügung, die jedoch selbst mit Nebenwirkungen und potentiellen Komplikationen vergesellschaftet sind. Bei Versagen konservativer Maßnahmen muss häufig auf mechanische Kreislaufunterstützungssysteme zurückgegriffen werden, die sich von der intraaortalen Ballonpumpe bis zur Implantation eines biventrikulären Assist-devices erstrecken. Kalziumsensitizer steigern die kardiale Kontraktilität, ohne die intrazelluläre Kalziumkonzentration zu beeinflussen und umgehen somit die Nebenwirkungen eines Kalziumüberangebots. Wir berichten von zwei Fällen ATV nach HTX, das sich jeweils nach der Gabe von Levosimendan, einem Kalziumsensitizer, rasch erholte.
Abstract
Heart transplantation is considered nowadays the gold standard in the therapy of the chronic and terminal heart insufficiency. Primary organ failure after heart transplantation is a severe complication generally related to poor quality of the organ, prolonged ischemia time, and acute rejection, all factors potentially leading to multi-organ failure. Pharmacological and mechanical support for these patients is limited and often related to side-effects. Ca2+ sensitisers have been proposed to increase cardiac contractility without altering the intracellular Ca2+, thus avoiding the Ca2+ overload side-effects. We report two cases of heart transplanted patients suffering from primary organ failure in the early postoperative period who recovered after i.v. administration of levosimendan, a Ca2+ sensitiser.
