Ziel

Darstellung und Analyse klinikspezifischer Methoden zur Förderung der Transparenz und Qualitätskontrollen herzchirurgischer Leistungen.

Methoden

Unser Vorgehen umfasst:

1. die zeitnahe Offenlegung herzchirurgischer Ergebnisse mittels

a) automatischer Herausgabe des OP-Berichtes noch am Operationstag.

b) Versenden detaillierter Verlaufsberichte im Falle des Auftretens von Komplikationen.

c) Herausgabe einer jährlichen Operations- und Komplikationsstatistik an alle Einweiser und interessierte Patienten.

2. Kontinuierliche Erhebung und Auswertung von Follow-up-Daten.

3. Aufbau eines ‚Datamart‘ basierten abteilungsübergreifenden Informationssystems.

4. Entwicklung klinik-spezifischer Methoden der Risikoeinschätzung.

5. Web basiertes Informationsportal im Intranet für die Ärzte mit aktualisierter Darstellung Risiko-adjustierter Ergebnisse und ‚Variable life Adjusted Displays‘ (VLADs).

6. Komplikationskonferenzen.

Ergebnisse

Die automatisierte und tagesaktuelle Herausgabe der Operations- und Verlaufsberichte erleichtert den externen Ärzten und Verwandten den Einblick in den Behandlungsablauf. Die Methode der Follow-up-Erhebungen ist effektiv (30-tages-Follow-up >99%, Intermediate-Follow-up >90%) und kostet ca. € 10,–/Patient. Die Verknüpfung dieser ausführlichen Daten zum Behandlungsverlauf mit den reichhaltigen perioperativen Daten im ‚Datamart‘ schafft eine Voraussetzung risiko-adjustiert Behandlungsergebnisse flexibel zu berechnen. EuroSCORE basierte Vorhersagen erreichen gute bis sehr gute Vorhersagewerte (c-Index 0,75 bis 0,79) und können für die Kalkulationen externer und interner Standards verwendet werden. Über die Intranet basierten VLADs wird die Performance der Klinik, einzelner Operateure oder anderer Subgruppen kontinuierlich dargestellt.

Schlussfolgerung

Mit Hilfe der hier vorgestelltenMethoden wird die Transparenz herzchirurgischer Leistungen gefördert und die Voraussetzungen für interne und externe Qualitätskontrollen geschaffen.

To evaluate clinic specific methods for handling of information and quality assessment in cardiac surgery.

Methods

The clinic specific techniques include:

1. actual reports sent to patients and doctors, including the following

a) Surgical protocols dismissed on the day of operation.

b) Detailed reports on patients with prolonged hospital stay and

c) Annual statistics including data of adverse events.

2. Routine 30 day and intermediate follow-up.

3. Establishing of a multi-purpose database (‘datamart’).

4. Development of clinic specific models for risk adjusted outcome analysis.

5. Web based information portal available for physicians for comfortable data export and access to actual outcome statistics and Variable Life Adjusted Displays (VLADs).

6. Weekly meetings of our physicians for joint analysis of adverse outcomes.

Results

Family doctors, cardiologists and relatives of the patients have day-to-day information about surgical therapy and the perioperative course of the patient, which facilitates joint care. The 30-day follow-up and intermediate follow-up reveal excellent results (> 99 resp. >90%). Easy availability of multiple perioperative data in the ‘Datamart’ and accurate follow-up data enhance risk-adjusted outcome analysis. EuroSCORE based estimation of early mortality shows good results (c-index of 0.75 to 0.79) and allows the calculation of external and internal standards. Intranet based VLADs demonstrate the performance of the clinic, individual surgeons and other arbitrary groups.

Conclusion

The methods introduced here, promote the utmost transparency towards patients, referring physicians and medical centers. Furthermore they are prerequisites for internal and external quality control measures.
