Zusammenfassung
Sozioökonomische Untersuchungen werden von einer zunehmenden Zahl europäischer Gesundheitsbehörden bei der Diskussion um die Erstattungsfähigkeit von Medikamenten berücksichtigt. Innerhalb rheumatologischer Fachkreise hat sich unter dem Dach von OMERACT (Outcome Measures in Rheumatoid Arthritis Clinical Trials) eine Initiative gebildet, die bei dem für 2000 geplanten OMERACT Treffen in Toulouse den Transfer sozioökonomischer Standards in die Rheumatologie behandeln wird. Hinsichtlich der Kosten-effektivität der Cox-2-Inhibitoren ist zunächst auf umfangreiche Vorarbeiten aus der Diskussion der Misoprostolprophylaxe zu verweisen. Weiterhin liegen zwei Modellrechnungen zur Beurteilung der Kosten-effektivität der Cox-2-Inhibitoren vor: eine publizierte kanadische Untersuchung (Vergleich Nabumeton versus Naproxen) sowie eine bislang nicht publizierte Untersuchung zur Kosteneffektivität von Celecoxib (ACCES: Arthritis Cost Consequence Evaluation System). Deutsche Daten weisen gastrointestinale Blutungen mit 32,9% als häufigste unerwünschte Arzneimittelwirkungen, die zu einer stationären Aufnahme führen, auf. Weitere Daten, die die Morbidität infolge der unerwünschten Arzneimittelwirkungen von nicht-steroidalen Antirheumatika erfassen, sind erforderlich. Auf der Grundlage solcher Daten könnte dann die Vermeidung nebenwirkungsbedingter Morbidität und Mortalität sowie das daraus sich ergebende Einsparungspotentials bei Einsatz der selektiven Cox-2-Inhibitoren in Deutschland quantifiziert werden.
Summary
Health authorities of several European countries recently introduced guidelines for socioeconomic evaluations. Additionally the activities of OMERACT (Outcome Measures in Rheumatoid Arthritis Clinical Trials) indicate an increasing awareness for the need of socioeconomic studies in rheumatology. The planned 2000 OMERACT meeting in Toulouse will address transfer of socioeconomic standards into rheumatology. In terms of cost effectiveness of selective Cox-2 inhibitors, a reference has to be made to the preceeding discussion of cost effectiveness of Misoprostol. In addition, there are two models examining the cost effectiveness of Cox-2 inhibitors: a Canadian model comparing Nabumetone and Naproxen and an unpublished model assessing the cost effectiveness of Celecoxib (ACCES: Arthritis Cost Consequence Evaluation System). German data indicate that gastrointestinal bleedings account for 32.9% of all adverse drug events leading to a hospital admission. Further data assessing the morbidity due to adverse effects of nonsteroidal anti-inflammatory drugs are needed. Such data would allow the quantification of possible savings related to the usage of Cox-2 inhibitors in Germany.
