Zusammenfassung
Epidemiologische Daten der Kerndokumentation der regionalen kooperativen Rheumazentren in Deutschland belegen, dass das Symptom Schmerz sowohl bei aktiver als auch in Remission befindlicher entzündlich rheumatischer Erkrankung ein immenses Problem darstellt. Per Definition handelt es sich beim Schmerz um eine unerfreuliche, sensorische und emotionale Erfahrung. Im Rahmen von entzündlich rheumatischen Erkrankungen wird der nozizeptive Schmerz durch Entzündungsmediatoren oder strukturell mechanische Fehlbelastungen über eine Aktivierung der Nozizeptoren ausgelöst. Zudem wird die Entzündung auch durch Freisetzung von proinflammatorischen Substanzen wie z. B. Substanz P oder „calcitonin gene-related peptide“ (CGRP) aus aktivierten Nozizeptoren lokal verstärkt. Die Phänomene Entzündung und Schmerz sind in einem selbstverstärkenden Mechanismus miteinander verbunden, was erklärt, warum Schmerz durch Entzündungshemmung wirksam bekämpft werden kann, und umgekehrt, warum Schmerzhemmung auch positive Effekte auf die Entzündungsreaktion mit sich bringt. Diese Übersichtsarbeit fokussiert auf Publikationen zur multimodalen rheumatologischen Komplexbehandlung und Thermotherapie, die unter evidenzbasierten Studienkriterien eine Schmerzminderung sowie zum Teil Beeinflussung molekularer Marker aufzeigen mit konsekutivem Einfluss auf die Schmerzentstehung und das Entzündungsgeschehen. Die Resultate werden im Kontext mit aktuellen physiologischen Erkenntnissen zur Schmerzentstehung dargestellt.
Abstract
Epidemiological data from the core documentation of the regional cooperative rheumatism centers in Germany confirm that the symptom of pain is an immense problem in both active inflammatory rheumatic diseases and when in remission. By definition pain is an unpleasant sensory and emotional experience. In the context of inflammatory rheumatic diseases, nociceptive pain is triggered by inflammatory mediators or structurally mechanical distress via activation of the nociceptors. In addition, inflammation is also locally enhanced by the release of proinflammatory substances, such as substance P or calcitonin gene-related peptide (CGRP) from activated nociceptors. The phenomena of inflammation and pain are linked in a self-reinforcing mechanism, which explains why pain can be effectively controlled by inhibiting inflammation and, conversely, why inhibiting pain also has positive effects on the inflammatory response. This review focuses on publications on multimodal rheumatological complex treatment and thermotherapy, which, under evidence-based study criteria, showed a reduction in pain and a partial influencing of molecular markers with a subsequent influence on the development of pain and the inflammatory process. The results are presented in the context of current physiological knowledge on the development of pain.
