Zusammenfassung
Die Sarkoidose ist eine Systemerkrankung, die durch nichtverkäsende Granulome der Lunge und in anderen Organen charakterisiert ist. Bei einer Daktylitis sollte nach auffälligen Befunden der Augen, der Haut und der Lungen gesucht werden. Auch bei unauffälligen ACE-Werten kann die Verdachtsdiagnose einer Sarkoidose gestellt und durch eine Gewebebiopsie bestätigt werden. Unter Therapie mit Methotrexat und Steroiden kann eine anhaltende Stabilisierung der Krankheitsaktivität erreicht werden. Bei refraktären Verläufen können zusätzlich Leflunomid, Azathioprin, Hyroxychloroquin oder Antikörper gegen TNF-α eingesetzt werden.
Abstract
Sarcoidosis is a systemic granulomatous disease that primarily affects the lung and other organs. Patients with dactylitis should be screened for abnormalities of the skin, eyes and lungs. Even in patients with normal-range ACE sarcoidosis could be confirmed by tissue biopsy. Treatment with methotrexate and steroids can relieve symptoms and stabilize the disease. In refractory cases leflunomide, azathioprine, hydroxychloroquine or antibodies against TNF-α can be additionally administered.
