Zusammenfassung
Die Behandlung der symptomatischen koronaren Mehrgefäßerkrankung mit Ballondilatation (PTCA) oder Bypass-Operation ist nach den Ergebnissen von 5 großen, randomisierten Studien mit vergleichbarem klinischen Erfolg und Risiko möglich. Nach PTCA ist allerdings die Zahl der Reinterventionen deutlich höher. Im Langzeitverlauf war nach der BARI-Studie bei diabetischen Patienten die Letalität in der operierten Gruppe geringer. Die individuelle Therapieempfehlung durch den behandelnden Arzt hat sich ebenfalls als wichtiger Faktor für die Prognose des Patienten erwiesen. Der Einfluß der neueren Techniken (Stents) wird derzeit in weiteren groß angelegten randomisierten Studien untersucht.
Summary
Treatment of symptomatic multivessel coronary disease with balloon angioplasty (PTCA) or bypass surgery as based on the results of 5 large, randomized trials is equivalent with respect to clinical outcome and risk. After PTCA, however, the rate of reinterventions is considerably higher. During long-term follow-up in the BARI trial, mortality was lower in diabetic patients undergoing surgery. Individual therapeutic decision making by the treating physician proved to be an important factor for the patients prognosis. The role of new devices (stents) is currently under investigation in new large randomized trials.
