Zusammenfassung
Eine 66jährige Patientin mit pulmonaler Sarkoidose hatte eine eineinhalbjährige Anamnese einer rezidivierenden Linksherzinsuffizienz. Klinisch bestand das Bild einer dilatativen Kardiomyopathie. Endomyokardiale Biopsien zeigten initial eine lymphozytäre Myokarditis, bei erneuter Biopsie jedoch ein epitheloidzelliges Granulom ohne Nekrose vom Typ der Sarkoidose. Die Patientin wurde neben der Standardtherapie mit Kortikosteroiden behandelt.
Diese Fall unterstreicht die Schwierigkeit und Wichtigkeit der Diagnosesicherung einer kardialen Sarkoidose als eine potentiell behandelbare Form der dilatativen Kardiomyopathie.
Summary
A 66-year-old woman with pulmonary sarcoidosis had a 1.5-year history of congestive heart failure presenting as dilated cardiomyopathy. Transvenous endomyocardial biopsy specimens initially revealed lymphocytic myocarditis but subsequently showed a non-cesating giant-cell granuloma typical of Boeck's sarcoid. In addition to standard therapy the patient was given corticosteroids.
This case illustrates the difficulties and importance of diagnosing cardiac sarcoidosis as a potentially treatable form of dilated cardiomyopathy.
