Stenttherapie bei diabeteskranken Koronarpatienten

Stent therapy with diabetic patients who have coronary artery disease

Summary

When bare-metal stents are used, diabetic patients with coronary artery disease have a poorer prognosis than non-diabetic patients after coronary interventions. In clinical studies, it could be demonstrated that the disadvantage for diabetic patients is not present when drug-eluting stents are used. Today, two types of drug-eluting stents are clinically used: the sirolimus-eluting stent (Cypher™) and the paclitaxel-eluting stent (Taxus™). Even the high risk group of patients with in-stent stenosis can be efficiently treated with drugeluting stents.

Zusammenfassung

Koronarkranke Diabetiker haben grundsätzlich eine schlechtere Prognose als Nichtdiabetiker nach perkutanen Koronarinterventionen, wenn nichtbeschichtete Koronarstents verwendet werden. In klinischen Studien konnte gezeigt werden, dass bei Verwendung von so genannten Medikamenten eluierenden Stents dieser Nachteil für Diabetiker nicht mehr besteht. Zurzeit sind 2 Typen von Medikamenten eluierenden Stents untersucht. Es handelt sich um einen Sirolimus eluierenden Stent (Cypher™) sowie einen Paclitaxel eluierenden Stent (Taxus™™). Selbst in der Hochrisikogruppe der Instentrezidivpatienten mit Diabetes ist die Therapie mit Medikamenten eluierenden Stents effizient und einer Bypassoperation vorzuziehen.
