Summary

A variety of randomized, controlled trials comparing percutaneous coronary intervention (PCI) with coronary artery bypass grafting (CABG) found similar results in mortality but significant differences in number of reinterventions in favor of CABG. This work gives an overview about the relevance and limitations of these studies in line with newly published large scale observational studies, which reveal significantly lower mortality-rates in CABG patients. Emphasis is placed on the special situation in the diabetic patient.

Zusammenfassung

Eine Vielzahl randomisierter, kontrollierter Studien haben bei dem Vergleich zwischen interventioneller Myokardrevaskularisation (PCI) und Koronarchirurgie (CABG) größtenteils geringe Unterschiede im Überleben bei jedoch signifikanten Unterschieden in den Reinterventionsraten zugunsten der operativen Strategie gefunden. Diese Arbeit gibt einen Überblick über die Bedeutung einschließlich Limitierung dieser Untersuchungen vor dem Hintergrund neu publizierter groß angelegter Observationsstudien, die signifikante Vorteile für CABG auch in der Mortalität sehen. Die besondere Situation des Diabetikers wird dabei gesondert beleuchtet.
