Heparininduzierte extrakorporale LDL-Präzipitation (H.E.L.P.)

Summary.

The H.E.L.P procedure (heparin-induced extracorporeal LDL precipitation) is a selective and sophisticated apheresis procedure. By means of heparin and lowering the pH level, lipoproteins and fibrinogen are reduced by 50–60%. In addition to lipoprotein (a) reduction (50– 60%) adhesion molecules (ICAM- 1, VCAM-1, P selectin), which play a major role in the development and progression of atherosclerosis, are significantly lowered.

Within 20 hours after LDL apheresis, positron emission tomography was able to verify an improvement of coronary vasodilation capacity. This is likely to result from a considerable reduction of LDL cholesterol and fibrinogen with a consecutive improvement of both endothelial function and rheology.

Zusammenfassung.

Bei dem H.E.L.P-Verfahren (heparininduzierte extrakorporale LDL-Präzipitation) handelt es sich um ein selektives und schonendes Apherese-Verfahren. Mittels Heparin und einer pH-Wert-Senkung werden Lipoproteine und Fibrinogen um 50–60% gesenkt. Neben der Lipoprotein (a)-Senkung (50–60%) werden auch Adhäsionsmoleküle (ICAM-1, VCAM-1, P-Selectin), denen eine Schlüsselrolle in der Entstehung und Progression der Atherosklerose zukommt, deutlich gesenkt.

Nach einer LDL-Apherese lässt sich innerhalb von 20 h durch PET-Untersuchungen eine Verbesserung der koronaren Vasodilatationskapazität nachweisen. Dies dürfte in hohem Maße auf die bedeutsame Senkung des LDL-Cholesterins und Fibrinogens mit konsekutiver Verbesserung der Endothelfunktion und Rheologie zurückzuführen sein.
