Summary.
The implantation of transvenous devices in patients who underwent tricuspid valve replacement represents problems, especially if an epicardial position is not available. The implantation of a “pace-sense” lead via the coronary sinus is a safe and feasible procedure. For experienced surgeons in implantation of biventricular devices, the implantation of leads via the coronary sinus is a routine procedure. Bipolar leads are essential for the correct sensing and pacing of the implantable cardioverter-defibrillator (ICD).
In patients who underwent tricuspid valve replacement who have the indication of an ICD implantation postoperatively, the combination of a shock electrode placed in the superior vena cava, a subcutaneous array positioned on the left posterior close to the spine and an active can, placed subpectorally in the left infraclavicular region, is an alternative solution.
Zusammenfassung.
Patienten nach einem chirurgischen Trikuspidalklappenersatz stellen für die transvenöse Implantation von Schrittmachern oder Defibrillatoren ein Problem dar, insbesondere bei einem Ausschluss des epikardialen Zuganges. Die Implantation einer “Pace-Sense”-Elektrode über den Sinus coronarius ist sicher durchzuführen und stellt eine gute Alternative dar. Für Implanteure, die im Rahmen der biventrikulären Schrittmacherchirurgie Erfahrungen gesammelt haben, ist der Zugang zum Koronarvenensinus Routine. Für eine korrekte Erkennung und Stimulation ist die Implantation einer bipolaren Elektrode notwendig.
Die Kombination aus einer transvenös implantierten Schockelektrode in die obere Hohlvene und einer weit nach links dorsal bis zur Wirbelsäule vorgeschobenen subkutanen “Fingerelktrode”, in Kombination mit einem aktiven Gehäuses, welches links infraclavikulär subpectoral positioniert wird, erscheint für Patienten nach einem biologischen Trikuspidalklappenersatz, die einen ICD benötigen, eine gute Lösung.
