Spontane Koronararterien-Dissektion: seltene Ursache eines akuten Koronarsyndromes

Summary.

Spontaneous coronary artery dissection is a rare cause of an acute coronary syndrome. This report describes a previously healthy woman without cardiovascular risk factors who presented with an acute anterior non-ST elevation myocardial infarction. Coronary angiography revealed an isolated longitudinal dissection in the middle part of the left anterior descending coronary artery (LAD) with normal flow of the contrast media. The patient was treated conservatively with heparin, aspirin, clopidogrel, and beta-receptor blocker. Stress exercise test was normal at discharge. After an event-free follow-up of three and a half months coronary angiography showed a completely normal LAD. Literature about epidemiology, pathogenesis, diagnosis and treatment of spontaneous coronary artery dissection is reviewed.

Zusammenfassung

Die spontane Dissektion von Koronararterien ist eine seltene Ursache für ein akutes koronares Syndrom. Wir beschreiben den Fall einer bis anhin gesunden 38-jährigen Frau ohne fassbare kardiovaskuläre Risikofaktoren mit einem akuten anterioren Myokardinfarkt ohne ST-Hebung. Die koronarangiographische Untersuchung zeigte eine longitudinale Dissektion des Ramus interventricularis anterior im mittleren Drittel mit unbehindertem Kontrastmittelfluss in die Peripherie des Gefäßes. Die Patientin wurde konservativ behandelt mit Heparin, Aspirin, Clopidogrel, und Betablocker. Die Fahrrad-Ergometrie vor Spitalentlassung fiel unauffällig aus. Nach einem beschwerdefreien Verlauf von dreieinhalb Monaten war die Dissektion angiographisch nicht mehr nachweisbar. Anhand einer Literaturübersicht werden Epidemiolgie, Pathogenese und Therapie dieser seltenen Erkrankung diskutiert.
