Zusammenfassung
Osteoporotische Wirbelkörperfrakturen sind typische Altersfrakturen. Häufig werden sie in ihren Auswirkungen auf die Lebensqualität des Betroffenen unterschätzt. Ihre Prävention durch eine leitliniengerechte Osteoporosetherapie ist daher sehr bedeutsam. Die Unterscheidung zwischen frischen und alten Wirbelkörperfrakturen ist häufig nur im MRT (hier zeigt sich ein Ödem) möglich. Die für die traumatischen Frakturen des jüngeren Menschen entwickelten Therapieempfehlungen sind beim alten Patienten nur teilweise anwendbar. Eine neue Klassifikation als Grundlage für die Indikationsstellung einer operativen oder interventionellen Therapie wird in diesem Beitrag vorgestellt. Die Vertebro-/Kyphoplastie und auch die Frakturstabilisation mithilfe des Fixateur interne stellen minimalinvasive Therapiemethoden dar. Die Anwendung von Knochenzement sorgt für zusätzliche Stabilität, geht jedoch mit der Gefahr zementspezifischer Komplikationsmöglichkeiten einher (z. B. Zementaustritt, -embolie).
Abstract
Osteoporotic spinal fractures are typical age-related fragility fractures. Their impact on the quality of life is often underestimated; therefore, anti-osteoporotic drug treatment according to the current guidelines is essential. Occult vertebral fractures may be difficult to detect and a differentiation between fresh and old fractures is often only possible with magnetic resonance imaging (MRI) to reveal cancellous bone edema. Treatment recommendations are predominantly based on traumatic fractures in younger adults and are thus not applicable in orthogeriatrics. A new classification currently under validation and presented in this article was developed in order to aid decision-making for operative interventions. Minimally invasive treatment options include vertebroplasty, kyphopasty and internal fixation. The application of cement provides additional stability but can be accompanied by cement-specific complications, such as extravasation and embolism.

Die Applikation von Zement mit Wirbelkörperaufrichtung wird als Kyphoplastie bezeichnet.

Beim Einsatz eines Fixateur interne muss in jedem Fall der M. erector spinae von seinen Ansätzen an der Wirbelsäule abgetrennt werden.
Das Verfahren sollte nicht mit einer Vertebro-/Kyphoplastie kombiniert werden.
Auch bei Anwendung der MIS-Technik kann eine Zementaugmentation der Schrauben erfolgen.
Wenn eine Dekompression des Spinalkanals erreicht werden soll, sollte die MIS-Technik bevorzugt werden.
Die MIS-Technik wird auch als Spondylodese bezeichnet.
