Hämostasestörung bei Multiorgandysfunktion und Sepsis

Summary

Sepsis and septic shock are complex, dynamic and heterogeneous clinical syndromes. A systemic response to an infection can induce a cascade consisting of toxins – cytokines/mediators of inflammation – endothelial dysfunction – activation of blood coagulation – intravascular fibrin deposition – alteration of microcirculation, resulting in damage of organs. Multi-organ failure and sepsis are therefore closely connected. The associated disturbances of blood coagulation range between a simple activation of coagulation with a transient increase of activation markers (i.e., D-Dimer), similar to an acute-phase reaction, and full blown disseminated intravascular coagulation with consumption coagulopathy. This article summarizes the pathophysiological mechanisms of these disturbances, shows the necessary diagnostic tools and possible differential diagnosis, and discusses therapeutic options for sepsis and multi-organ failure.

Zusammenfassung

Sepsis und septischer Schock sind komplexe, dynamische und heterogene klinische Syndrome. Durch eine systemische Reaktion auf eine Infektion wird über die Achse Toxine – Zytokine/Entzündungsmediatoren – Endothelzellschädigung – Gerinnungsaktivierung – intravasale Fibrindeposition – Mikrozirkulationsstörung eine Schädigung von Endorganen hervorgerufen. Multiorganversagen und Sepsis stehen daher in engem Zusammenhang. Die dabei auftretenden Gerinnungsstörungen umfassen ein Spektrum von simpler Gerinnungsaktivierung mit passagerem Anstieg von Aktivierungsmarkern (D-Dimer), ähnlich einer Akutphasenreaktion, bis hin zur ausgeprägten disseminierten intravasalen Gerinnung mit Verbrauchskoagulopathie, die oft deletär endet. Dieser Artikel fasst die pathophysiologischen Mechanismen dieser Störungen zusammen, zeigt kurz die notwendige Labordiagnostik und die Differentialdiagnosen und diskutiert die verschiedenen Therapieoptionen der Gerinnungsstörungen bei Sepsis und Multiorgandysfunktion.
