Zusammenfassung
Der Abbau des Skelettmuskelproteins stellt ein herausragendes Merkmal bei kritischen Erkrankungen dar und wird als nützliche biologische Antwort auf diese angesehen. Der Muskelverlust ist verbunden mit einer Reihe von Problemen bei der medizinischen Versorgung und dieser Patienten. Es ist schwierig, den Proteinumsatz zuverlässig zu erfassen und der katabolische Zustand ist oft resistent gegen Behandlung, sei es mit Diät oder mit einer anderen Strategie. Diese Übersicht beschreibt das heutige physiologische Wissen auf diesem Gebiet, stellt die Sinnhaftigkeit der katabolen Antwort in Frage und betont einige neue Forschungsergebnisse, welche zu weiterer Forschung führen sollten.
Summary
Skeletal muscle protein breakdown is a dominant feature of critical illness and is believed to be a useful biological response to injury. However the loss of muscle is accompanied by some major difficulties in medical management and complications for the patient. Protein turnover is difficult to study and the catabolic state is resistant to treatment, whether by nutritional or other strategies. This article provides an overview of the current physiological knowledge in this area, questions the usefulness of the catabolic response and highlights some new research worthy of further investigation.
