Stentimplantation bei einem Koronararterienaneurysma Ein Fallbericht

Summary

The incidence of coronary artery aneurysm varies in the range from 0.2% to 4.9%. Ninety per cent of these were associated with atherosclerotic coronary artery disease, where vessel ectasia and poststenotic dilatations are not an infrequent finding. We present the case of a 67 year old patient with coronary artery aneurysm and coexisting coronary artery disease of the right coronary artery. The patient was symptomatic and had a posterolateral myocardial infarction. We performed a PTCA and stent-implantation of the stenosis and coronary artery aneurysm of the right coronary artery. After the procedure, the patient could be discharged without angina pectoris. A control-angiography will be performed six months later.

The definition, incidence, causes, complications, investigations, management, and prognosis of coronary artery aneurysms are discussed. Different management strategies are highlighted. The management of coronary artery aneurysms should be individualized depending on their location and the clinical context.

Zusammenfassung

Koronaraneurysmata kommen mit einer Inzidenz von 0,2 bis 4,9% selten vor. Sie sind zu 90% mit einer koronaren Herzerkrankung vergesellschaftet. In der vorliegenden Fallbeschreibung berichten wir über das therapeutische Vorgehen bei Koronararterienaneurysma der rechten Herzkranzarterie bei koexistenten signifikanten Koronarstenosen.

Bei dem symptomatischen 67jährigen Patienten wurde bei Z.n. Hinterwandinfarktgeschehen mit Thrombolysetherapie eine PTCA und Stentimplantation der rechten Koronararterie mit Beseitigung des Aneurymas durchgeführt. Der Patient konnte in beschwerdefreiem Zustand nach Hause entlassen werden. Eine invasive Kontrolldiagnostik ist nach einem Intervall von sechs Monaten geplant.

Diskutiert werden die Definition, Inzidenz, Ursachen, Komplikationen, sowie Management und Prognose von Koronaraneurysma. Weiterhin werden verschiedene therapeutische Strategien vorgestellt. Da die Datenlage bezüglich des therapeutischen Procederes derzeit unklar ist, sollte die Therapie individuell im Zusammenhang mit dem klinischen Kontext sowie abhängig von der Lokalisation des Aneurysmas und Koexistenz einer koronaren Herzerkrankung festgelegt werden.
