Summary
Metabolic acidosis is a primary disturbance in acid-base physiology characterized by either a gain of H+ or a loss of HCO– 3. The distinction is not trivial, since a gain in H+ is an emergency which may be life threatening. The anion gap, Na+–(Cl+HCO– 3), is very helpful in this regard, since gain of H+ disturbances are almost invariably non-chloride in nature. Thus, for every 1 mmol/L decrease in HCO– 3 an increase of 1 mmol/L in the anion gap would be expected. On the other hand, disturbances which involve loss of HCO– 3 via the gastrointestinal tract or which feature failure of the kidney to reclaim or generate HCO– 3 appropriately are characterized by increased renal Cl– reabsorption and result in a normal anion gap. We recently encountered a patient with severe metabolic acidosis who was misdiagnosed because his physicians could not make this distinction. The first steps in evaluating any metabolic acidosis should include calculating the anion gap. Further, since acid-base disturbances are frequently multifactorial, the anion gap should be calculated in all acid-base disturbances, irrespective of cause.
Zusammenfassung
Metabolische Azidose ist eine Störung, die entweder auf eine Zufuhr von starker Säure oder auf einen Verlust von HCO– 3 zurückzuführen ist. Der Unterschied ist nicht trivial, da Störungen, ausgelöst durch Zufuhr von starker Säure, öfter lebensgefährlich sind. Hier ist die Errechnung der Anionenlücke, Na+–(Cl– und HCO– 3), sehr hilfreich, da bei Zufuhr von starker Säure das begleitende Anion fast immer ein Nichtchloridanion ist. Für jeden mmol/l-Abfall im HCO– 3-Spiegel ist ein gleicher mmol/l-Anstieg in der Anionenlücke zu erwarten. Im Gegensatz dazu steigt bei Störungen, die durch den Verlust von HCO– 3 durch den Darm oder fehlender HCO– 3-Rückresorption oder Generation an der Niere ausgelöst sind, die Cl–-Rückresorption an der Niere mmol/l per mmol/l an. Die Anionenlücke bleibt dabei normal. Wir behandelten vor kurzem einen Patienten mit schwerer metabolischer Azidose, der durch fehlende Berücksichtigung der Anionenlücke von mehreren Ärzten falsch diagnostiziert wurde. In der Evaluierung jeder metabolischen Azidose sollte immer die Anionenlücke errechnet werden. Da Säure-Basen-Haushaltsstörungen häufig mehrere Ursachen haben, sollte die Anionenlücke auch bei jeder Säure-Basen-Haushaltsstörung berücksichtigt werden.
