Bedeutung phasenhaft auftretender nächtlicher Bradyarrhythmien

Summary

The occurrence of intermittent nocturnal bradycardia as well as bradyarrhythmias should lead to diagnosis of confounding sleep related breathing disorders (SRBD). From a therapeutic and prognostic point of view, a diagnosis with complete history, noninvasive and, if necessary, invasively cardiological investigations as well as cardiorespiratory polysomnography is essential. Apart from the exclusion or confirmation of a SRBD as the cause for nocturnal bradyarrhythmias, polysomnography enables the diagnosis of bradyarrhythmias which only occur during REM sleep. Bradyarrhythmias can usually be removed by effective therapy of the SRBD by nasal ventilation. It is still being discussed if patients with apnea-associated bradyarrhythmias require the implantation of a permanent pacemaker.

Zusammenfassung

Bei Patienten mit vorwiegend nächtlich auftretenden Bradykardien sowie bradykarden Herzrhythmusstörungen muß differentialdiagnostisch an das Vorliegen einer schlafbezogenen Atmungsstörung gedacht werden. Aus therapeutischen und prognostischen Gründen ist eine Diagnostik mit ausführlicher Anamnese, nichtinvasiver und ggf. invasiver kardiologischer Untersuchung sowie einer kardiorespiratorischen Polysomnographie notwendig. Neben Ausschluß oder Bestätigung einer schlafbezogenen Atmungsstörung als Ursache für nächtliche Bradyarrhythmien ermöglicht die Polysomnographie auch die Diagnostik ausschließlich REM-Schlafinduzierter Bradykardien. Mit der erfolgreichen Behandlung von SBAS durch die nasale Beatmungstherapie sind in der Regel auch die bradykarden Herzrhythmusstörungen eliminiert bzw. reduziert. Inwieweit Patienten mit ausschließlich apnoeassoziierten ventrikulären Asystolien aus prognostischer Sicht zusätzlich von der Implantation eines permanenten Schrittmachers profitieren, ist derzeit noch ungeklärt.
