Summary
Acute oxalosis is an important disorder causing serious abnormalities. Acute oxalosis may come about by ethylene glycol poisoning; in recent years iatrogenic cases of acute oxalosis have also been described following iv treatments of naftidrofuryl, a vasodilator. Furthermore, chronic asymptomatic intestinal oxalosis may turn symptomatic by a superimposed major load of oxalate. Acute oxalosis results in acute renal failure; in addition oxalate-induced angiopathy may be associated with necrosis of affected tissue. The diagnosis of acute oxalosis requires a demonstration of oxalate crystals in the urine and in other affected tissues. In terms of therapy, hemodialysis should be used promptly in cases of ethylene glycol poisoning and a worsening of the renal function. In cases of a secondary chronic intestinal oxalosis prophylactic measures such as maintaining an elevated urinary flow rate and prescribing oral calcium supplements will be helpful.
Zusammenfassung
Ein seltenes Krankheitsbild, die akute Oxalose, ist aufgrund seiner Symptomatik und der Möglichkeit einer iatrogenen Auslösung von intensivmedizinischem Interesse. Besondere Aufmerksamkeit verdienen die Intoxikationen mit oxalathaltigen Medikamenten. In der Vergangenheit war besonders der intravenös verabreichte Vasodilatator Naftidrofuryl aufgefallen. Auch Vergiftungen mit Äthylenglykol induzieren eine akute Oxalose. Außerdem können zunächst klinisch unauffällig verlaufende chronische Oxalosen, zum Beispiel die intestinale Form, durch eine zusätzliche Oxalatbelastung akut dekompensieren. Klinisch steht das akute Nierenversagen im Vordergrund, die Oxalat-Vasopathie kann jedoch zu weiteren schweren Komplikationen führen. Diagnostisch ist der Oxalatkristallnachweis im Urin und, wenn möglich, im Gewebe anzustreben. Die intensivmedizinische Therapie stützt sich auf eine strenge Überwachung der Nierenfunktion mit einer frühzeitigen Indikation zur Hämodialyse, welche bei Vergiftungen obligat ist. Bei der Möglichkeit einer klinisch latenten sekundären Oxalose, zum Beispiel nach einer Darmoperation, sind prophylaktische Maßnahmen sinnvoll.
