Abstract
Pacemaker emergencies are relatively rare; however, they can lead to life-threatening clinical situations. In emergencies, evaluation of clinical characteristics and exact analysis of a surface ECG is mandatory. There are “early” complications (within 6 months after pacemaker [PM] implant) and “late” complications observed during long-term follow-up. Among “early” complications, infection, perforation, hematothorax, pneumothorax and dislocation of electrodes are well known. Among “late” complications it seems helpful to identify bradycardias with or without PMstimulus artifacts (SA). Bradycardia without PM-SA are mainly caused by pacing or sensing problems and myopotentials, whereas bradycardia with PM-SA were frequently observed in patients with exit block, oversensing and pacemaker syndrome. In bradycardia- related pacemaker emergencies application of atropine (0.5 mg i.v.) or orciprenalin (0.25–0.5 mg i.v.) is necessary. If these strategies fail transvenous or transcutaneous external stimulation is necessary; cardiopulmonary resuscitation is mandatory when asystole is present. Tachycardias can be caused by implanted PM (“PMT”) or caused by other reasons. Diagnosis of PMT is relatively easy in patients with sequential PM using surface ECG. In these patients vagal maneuvers are the first therapeutic options. Interference in implanted cardiac devices are possible by direct electric energy and electromagnetic interference. Driving should only be limited for the first week after PM implantation.
Zusammenfassung
Notfallsituationen sind bei Schrittmacherpatienten relativ selten, können aber zu lebensbedrohlichen Situationen führen. Von entscheidender Bedeutung ist im Notfall die Erfassung der klinischen Symptomatik und eine genaue Analyse des Oberflächen-EKGs. Komplikationen werden „früh“ (< 6 Monaten nach Schrittmacher (SM)-Implantation oder erst im Langzeit-Verlauf („spät“)) beobachtet. Von den „frühen“ Komplikationen sind besonders Infektionen, Perforationen, Hämatothorax/Pneumothorax und Elektrodendislokationen gefürchtet. Bei den „späten“ Komplikationen sollten Bradykardien mit oder ohne Stimulationsartefakt Tachykardien gegenübergestellt werden. Beim Nachweis einer Bradykardie und fehlender SM-Stimulation liegt entweder eine Störung des Schrittmachersystems vor oder externe elektrische Phänomene durch Muskelpotentiale oder technische Geräte führen zu einer Inhibierung der Schrittmacherstimulation. Bradykardien mit SM Spike werden vor allem durch Exit-Block, Oversensing und SMSyndrom hervorgerufen. In der Notfallsituation kann bei symptomatischer Bradykardie versucht werden, die Herzfrequenz durch Applikation von Atropin (0,5 mg i.v.) oder Orciprenalin (0,25– 0,5 mg i.v.) zu steigern. Bei Versagen dieser medikamentösen Maßnahmen kann eine transvenöse oder transkutane Elektrostimulation notwendig werden, bei Asystolie muss eine sofortige kardiopulmonale Reanimation erfolgen. Tachykarde Rhythmusstörungen können durch den SM bedingt sein oder aber andere Ursachen haben. Die Diagnose einer SM-induzierten Tachykardie (PMT) ist bei Patienten mit sequentiellen SM-Systemen aus dem Elektrokardiogramm relativ einfach zu stellen. Als therapeutische Maßnahmen der ersten Wahl sind vagale Provokationsmanöver zu empfehlen. Störeinflüsse sind durch direkten Stromkontakt und/oder durch elektromagnetische Felder möglich und können zu Fehlfunktionen des SM führen. Eine Einschränkung des Fahrverhaltens ist nur für 1 Woche nach SM-Implantation gegeben.
