Zusammenfassung
Der Fall beschreibt den Verlauf eines medikamenteninduzierten Diabetes mellitus bei einem 36 Jahre alten männlichen Patienten. Er wurde im diabetischen ketoazidotischen Koma ins Krankenhaus eingeliefert. Nachdem die Serumglukosekonzentration (initial bei 1200 mg/dl) gesenkt war, blieb die Ätiologie des Diabetes unklar, und der Patient war mit bis zu 80 IE Altinsulin pro Tag weiterhin insulinpflichtig. Das klinische Bild war weder für einen Diabetes mellitus Typ 1 noch für einen Diabetes mellitus Typ 2 typisch. Aufgrund einer psychotischen Störung war der Patient seit etwa 2 Jahren mit Clozapin eingestellt. Eine seltene Nebenwirkung dieses Medikaments ist die Entstehung eines Diabetes mellitus. Nach der Umstellung auf ein anderes Neuroleptikum senkte sich der Insulinbedarf innerhalb von 4 Tagen auf 0. Da der Krankheitsverlauf sich stark mit anderen beschriebenen Fällen deckt ist ein direkter Zusammenhang zwischen einer Therapie mit Clozapin und der Entstehung des Diabetes mellitus im beschriebenen Fall sehr wahrscheinlich.
Abstract
The case of clozapine-induced diabetes mellitus in a 36-year-old male patient is described. He was delivered to the hospital in a comatose state due to diabetic ketoacidosis. After serum glucose concentration (initially 1200 mg/dl) was lowered, the etiology of the diabetes mellitus was not clear and the patient still required up to 80 IU insulin per day. The symptoms were neither typical for type 1 nor for type 2 diabetes mellitus. Due to a psychotic disturbance, the patient had been treated with clozapine for about two years. A rare complication of this treatment is the development of diabetes mellitus. After changing to another neuroleptica, the need for insulin decreased to zero within four days. Because of congruence with other described cases, a direct dependence between treatment with clozapine and the induction of the diabetes can probably be expected.
