Summary
A 69-year old woman was admitted to the hospital with progradient chest pain for two weeks. Within the last few days, left thoracic pain increased and was also now localized in the back. There were no diagnostic evaluation studies performed. At admission, there was a diastolic murmur due to a moderate aortic insufficiency (AI) and ST-segment depression in V2–V6 (0.1 mV) in the 12-lead ECG. Troponin was negative and further risk markers were not present. In the transthoracic echocardiogram the AI was visible and the cardiac magnet resonance imaging (MRI) clearly showed in the True Fisp cine sequences a dissected membrane in the ascending aorta. The patient was transferred to the intensive care unit and was admitted immediately to the cardiac surgery department. During surgery a prosthetic aortic valve and a prosthetic ascending aorta were implanted. Since the operation, the patient had no symptoms and is already at home.
Zusammenfassung
Eine 69-jährige Patientin wird wegen seit 2 Wochen bestehender progredienter thorakaler Schmerzen aufgenommen. Innerhalb der letzten Tage waren die linksthorakalen Schmerzen stärker geworden und strahlten nun auch in den Rücken aus. Eine Abklärung der Symptomatik war bisher nicht durchgeführt worden bzw. ergebnislos verlaufen. Bei Aufnahme fand sich auskultatorisch eine mittelschwere Aorteninsuffizienz (AI), im EKG wurden descendierende ST-Strecken-Senkungen in V2–V6 von 0,1 mV nachgewiesen, Troponin war negativ, andere Risikomerkmale lagen nicht vor. Während in der transthorakalen Echokardiographie die AI gesehen wurde, konnte in den True Fisp Cine-Aufnahmen der Kernspintomographie des Herzens eine Dissektionsmembran in der Aorta ascendens dargestellt werden. Die Patientin wurde umgehend auf die Intensivstation gebracht und unverzüglich zur Operation verlegt. Nach Aortenklappenersatz (21 mm Doppelkippscheibenprothese) sowie Ersatz der Aorta ascendens mittels eines klappentragenden ATS Conduits wurde die Patientin in gutem Allgemeinzustand zurückverlegt. Postoperativ war die Patientin vollkommen beschwerdefrei und ist inzwischen wieder zu Hause.
