Herzrhythmusstörungen: Monomorphe ventrikuläre Extrasystolie nach Stromunfall

Summary

Accidents with electrical current are common in industry, construction and at home. During construction work, a 39-year old male briefly touched an electric wire (ca. 1 second, 230 V alternating current). For the duration of the direct contact, the patient had severe pain in his left arm and chest and paralysis of his left arm. A milder chest pain persisted for 30–45 minutes. An emergency physician examined the patient following the accident and discovered a newly developed extrasystoly that was confirmed as monomorphic ventricular extrasystoly by ECG. The monomorphic ventricular extrasystoly could still be documented 8 months later by Holter ECG. On follow-up 12 months after the accident, no arrhythmia could be found. Local myocardial damage due to current flow through the heart is the probable cause of the extrasystoly.

Zusammenfassung

Elektrische Unfälle gehören zu den häufigen Unfällen in Industrie, Baugewerbe und Haushalt. Während Montagearbeiten kam es bei einem 39-jährigen Patienten zu einer kurzen Berührung (Kontaktzeit ca. 1 Sekunde) mit einer stromführenden Leitung (230 Volt Wechselstrom). Während des Stromkontaktes hatte der Patient einen stechenden Schmerz in der Brust und im linken Arm, sowie eine Paralyse des Armes. Der Brustschmerz hielt abgeschwächt für 30–45 Minuten an. Bei der am Unfalltag durch den D-Arzt durchgeführten klinischen Untersuchung fiel eine neu aufgetretene Extrasystolie auf. Im EKG ließ sich eine monomorphe ventrikuläre Extrasystolie dokumentieren. Diese monomorphe Extrasystolie konnte auch noch nach 8 Monaten im Langzeit-EKG nachgewiesen werden. Zwölf Monate nach dem initialen Ereigniss bestand die monomorphe Extrasystolie nicht mehr. Als Pathomechanismus dieser Herzrhythmusstörung ist eine – durch den Stromfluss bedingte – lokale myokardiale Schädigung anzunehmen.
