Summary
Severe bacterial infections represent a major cause of high mortality on intensive care units. In view of the fact that the responsible pathogen is in most cases unknown prior to therapy, the question arises whether the microbiological results are relevant for the outcome of the patients. On the other hand, response to antibiotic therapy has to be reliably evaluated during the treatment and an appropriate objective method needs to be established. Therefore, we prospectively investigated in a cohort study of 2×20 patients (group A: n=20 with positive cultures; group B: n=20 with negative cultures) the relevance of microbiological results for therapy outcome. The therapy effect was evaluated by the clinical course as well as by CRP changes. At the same time we investigated the intensity of therapy by the Therapeutic Intervention Scoring System (TISS). The classification of illness severity was documented by the Mortality Probability Model (MPM). In the course of therapy the CRP together with the necessary TISS workload were significantly reduced (p<0.05) within each group from the second to the seventh therapy day. Comparison between the groups did not show relevant differences: decrease of CRP was 75.7% in group A and 64.1% in group B. Therapeutic interventions scored by TISS were reduced by 15% in group A and by 17.5% in group B. The in-hospital mortality was 20% (n=4) in group A and 15% (n=3) in groupB, respectively. The combined monitoring of CRP changes and TISS changes has proved to be a reliable instrument to evaluate and to control the efficiency of antibiotic therapy already during the actual period of intensive care therapy. The study demonstrates that microbiological results did not affect the outcome and the necessary TISS workload in patients who received a calculated antibiotic therapy. Thus, the study underlines the importance of an initial effective therapy prior to the knowledge of the causative pathogen.
Zusammenfassung
Schwere bakterielle Infektionen sind selbst unter intensivmedizinischer Behandlung immer noch mit einer hohen Letalität verbunden. In Anbetracht der Tatsache, dass der verursachende Erreger zu Therapiebeginn meistens nicht bekannt ist, stellt sich die Frage nach der Bedeutung der mikrobiologischen Befunde für den Erfolg der Antibiotikatherapie. Außerdem muss das Ansprechen der Therapie bereits während der Behandlung zuverlässig erfasst werden. Hierzu bedarf es einer durchführbaren objektiven Methode. Deshalb untersuchten wir in Form einer Kohortenstudie prospektiv an 2×20 Patienten (Gruppe A=20 Fälle mit Erregernachweis und Gruppe B=20 Fälle ohne Erregernachweis) die Bedeutung der mikrobiologischen Befunde für das Therapieergebnis. Der Therapieeffekt wurde sowohl durch den klinischen Verlauf als auch durch die CRP-Dynamik bewertet. Gleichzeitig bestimmten wir den Behandlungsaufwand mittels Therapeutic Intervention Scoring System (TISS). Die Schweregradklassifikation erfolgte nach dem Mortality Probability Model (MPM). Im Verlauf der Therapie waren das CRP und ebenfalls der Aufwand nach TISS innerhalb beider Gruppen vom zweiten bis zum siebenten Behandlungstag signifikant rückläufig (p<0,05), zwischen den Gruppen A und B bestanden keine relevanten Unterschiede: Das CRP fiel bei A um 75,7% und bei B um 64,1%. Gleichzeitig reduzierte sich der erforderliche Aufwand nach TISS bei A um 15% und bei B um 17,5%. Die Krankenhaus-Letalität betrug in der Gruppe A 20% (4Patienten) und in der GruppeB 15% (3 Patienten). Die Kombination von CRP und TISS im Verlauf stellt ein geeignetes Kontrollinstrument zur Effizienz-Beurteilung der Antibiotikatherapie bereits während der aktuellen Intensivtherapie dar. Die Studie zeigt auf, dass eine kalkulierte Antibiotikatherapie bei Patienten, bei denen kein Erregernachweis gelingt und damit eine im Therapieverlauf gezielte Antibiose nicht möglich ist, den gleichen Effekt hat wie eine anfänglich kalkulierte Antibiotikatherapie, die nach dem Vorliegen des Erregernachweises gezielt fortgeführt wird. Wesentlich für ein gutes Outcome ist die unabhängig vom Erregernachweis initial eingeleitete effektive Therapie.
