Transsklerale und transpupilläre Laserkoagulation bei proliferativer Sichelzellretinopathie

Zusammenfassung

Hintergrund. Die transsklerale Diodenlaserkoagulation ist ein neues Verfahren zur Therapie der proliferierenden Sichelzellretinopathie (PSR). Sie erweitert das Therapiespektrum der PSR, das bisher aus transpupillärer Laser- und transskleraler Kryokoagulation bestand.

Fallbericht. Es wird über 2 Patienten mit Sichelzellerkrankung und PSR berichtet, von denen eine Patientin mit dem Argonlaser transpupillär und ein weiterer Patient mit dem Diodenlaser transskleral erfolgreich therapiert wurden.

Schlussfolgerung. Sowohl transpupilläre als auch transsklerale Laserkoagulationen sind bei der Therapie der PSR sicher und effektiv. Die transsklerale Diodenlaserkoagulation hat den Vorteil, auch bei Trübung der optischen Medien (z. B. bei Katarakt, Glaskörperblutung) oder bei medikamentös nicht erweiterbarer Pupille, eingesetzt werden zu können.

Abstract

Background. Transscleral diode laser photocoagulation is a new method for treating proliferative sickle-cell retinopathy (PSR). Other treatments include transpupillary laser photocoagulation and transscleral cryocoagulation

Case report. We report two patients with sickle-cell disease and PSR, one treated by transpupillary argon laser coagulation and the other by transscleral diode laser scatter photocoagulation.

Conclusion. Transpupillary and transscleral laser photocoagulation are effective and safe in the treatment of PSR. Transscleral photocoagulation is an alternative treatment method in eyes with PSR and cloudy optical media or with poor mydriasis in which transpupillary coagulation cannot be performed.
