Zusammenfassung
Hintergrund. Bei der überwiegenden Mehrzahl der Fälle extremer divergenter Schielstellungen handelt es sich um sekundäre Strabismusformen. Diese sind entweder Verletzungsfolgen, Anisometropien oder konsekutive Exotropien nach kombinierten Konvergenzoperationen. Für die operative Behandlung ergibt sich regelhaft folgendes Dilemma: Die Abweichung ist oft zu groß, um mit alleiniger konventioneller Umlagerung am Rectus medialis und lateralis am betroffenen Auge vollständig korrigiert zu werden, und die Patienten lehnen andererseits – mit gutem Grund – eine Operation auch am Führauge bei der häufig eher ästhetischen als funktionellen Indikation ab.
Methode. Vorgestellt wird ein Operationsverfahren, bei dem im Anschluss an die Muskelumlagerung der Effekt durch eine freie Schleimhauttransplantation verstärkt und gesichert wird.
Ergebnis. Mit dem vorgestellten Verfahren gelingt es zuverlässig, auch extreme Abweichungen (bis 45° entsprechend 90 pdpt) durch umlagernden Eingriff an nur einem Auge dauerhaft zu korrigieren.
Schlussfolgerung. Für die beschriebenen Patienten, denen oft fälschlicherweise gesagt wird, ihre Abweichung sei nicht zu korrigieren, stellt das vorgestellte Verfahren eine neue Option zur Verfügung.
Abstract
Background. The majority of cases of exotropia with extreme deviations (30–45° or 60–90 prism dioptres) are secondary forms of strabismus, either consecutive, following monolateral combined recession/resection procedures in childhood, extreme anisometropia or following trauma. For the surgical treatment the deviation is often too great to be corrected completely by conventional repositioning of the medial and lateral rectus muscle of the affected eye and patients justifiably refuse an operation even on the main eye because the indications are mostly more aesthetic than functional.
Methods. The paper presents a technique of a free mucosa transplant. The effect can be strengthened and made more secure following repositioning of the muscle.
Results. With this method even extreme deviations (up to 45° corresponding to 90 prism dioptres) could be successfully and permanently corrected by surgery on the deviated one eye only.
Conclusions. This method provides a new option for patients who are often incorrectly told that their deviation cannot be corrected by monolateral surgery.
