Nachdem das Fibrinolytikum rt-PA in der systemischen Therapie von arteriellen und venösen Gefäßverschlüssen erfolgreich eingesetzt wird, steht uns nun dieses Medikament als Injektionslösung zur lokalen Applikation im Auge zur Verfügung. rt-PA wandelt Plasminogen in Plasmin um, Plasmin lysiert Fibrin und Fibrinclots. Zur Fibrinbildung im Auge kann es durch intraokulare Eingriffe, Traumata oder entzündliche Prozesse kommen. In allen Fällen stellt Fibrin eine Komplikation dar, die in der Regel mit hochdosierten Steroiden nur langsam und oft unzureichend behandelt werden kann.
Patienten: Bei 8 Patienten im Alter zwischen 59 und 75 Jahren wurde bei Fibrinbildung nach kombinierter Goniotrepanation und Phakoemulsifikation mit HKL-Implantation 10 µg rt-PA intraokular injiziert. Am 1., 2. und 8. Tag nach der lokalen Fibrinolyse wurde der Verlauf des intraokularen Reizzustands mittels biomikroskopischem Befund, Fotodokumentation und Laser-Flare-Tyndallometrie beobachtet.
Ergebnisse: Bei sämtlichen behandelten Fällen kam es zur vollständigen Auflösung des Fibrinclots. Eine Rezidivbildung trat nicht ein, unerwünschte Nebenwirkungen zeigten sich nicht. Schlußfolgerung: Die intraokulare Anwendung von rt-PA führt zu einer sicheren Fibrinolyse und verhindert somit mögliche Spätkomplikationen aufgrund von Fibrinbildung nach kombinierter Glaukom- und Kataraktoperation.
Background: The fibrinolytic drug rt-PA, which has been successfully used systemically to treat arterial and venous occlusions, is now available for local, intraocular injection. rt-PA converts plasminogen to plasmin, which in turn is able to lyse fibrin and fibrin clots. Intraocular surgery, trauma and inflammation may lead to intraocular accumulation of fibrin. Treatment of this type of complication with high-dose corticosteroids is protracted and often unsuccessful. Patients: Eight patients (age 59–75 years) with fibrin formation following phacoemulsification and IOL implantation combined with trabeculectomy were treated by intra-ocular injection of 10 µg rt-PA. The intraocular status following the local fibrinolytic therapy was documented by biomicroscopy, photography and laser-flare Tyndallometry on days 1, 2 and 8.
Results: In all patients the fibrin clots dissolved completely. Neither relapses nor adverse affects were noted.
Conclusions: The intraocular application of rt-PA results in an efficient fibrinolytic effect, which prohibits potential complications of combined glaucoma and cataract surgery due to fibrin formation.
