Zusammenfassung
Die Kasuistik einer 82 Jahre alten Frau mit bilateraler nekrotisierender Fasziitis, davon einseitiger Orbitabeteiligung, wird vorgestellt. Es zeigte sich eine Mischinfektion durch Streptococcus pyogenes, Staphylococcus aureus und Aspergillus fumigatus. Letzterer wurde molekularpathologisch nachgewiesen. Therapeutisch kamen ein wiederholtes chirurgisches Débridement und eine intravenöse Breitbandantibiotikatherapie sowie Antimykotikatherapie zum Einsatz. Die Patientin erhielt zudem niedrig dosierte systemische Kortikosteroide. Nach Abklingen der Entzündungszeichen kam es zu einer unkomplizierten Wundheilung per secundam intentionem.
Abstract
The case of an 82-year-old woman with bilateral necrotizing fasciitis and unilateral orbital involvement is presented. A mixed infection consisting of Streptococcus pyogenes, Staphylococcus aureus, and Aspergillus fumigatus could be verified. The latter was detected by molecular pathological investigations. Treatment consisted of repeated surgical debridement in combination with systemic broad-spectrum antibiotic and antimycotic treatment. Furthermore, the patient received low-dose systemic corticosteroids. After remission of acute inflammation, the healing process by secondary intention was uneventful.
