Zusammenfassung
Hintergrund
Ocriplasmin (Jetrea®) ist eine therapeutische Option für Patienten mit fokaler vitreomakulärer Traktion (VMT) mit oder ohne kleine durchgreifende Makulaforamen (FTMH) < 400 µm. Ziel dieser Arbeit war es, das Wirkprofil der pharmakologischen Vitreolyse mit Ocriplasmin auf die retinale Mikrostruktur zu erfassen.
Methodik
Wir haben die Mikrostruktur der Netzhaut anhand der klinischen Befunde und mithilfe des SD-OCT (spectral-domain optische Kohärenztomographie) Verfahrens vor und nach der intravitrealen Injektion mit Ocriplasmin bei 70 Patienten mit VMT analysiert. Die SD-OCT-Scans wurden dann unter Berücksichtigung der folgenden Kriterien ausgewertet: Größe des Makulaforamens, Makulaödem, subretinale Flüssigkeit (SRF), Änderungen in der Ellipsoidzone (EZ) und der externen Grenzmembran (ELM).
Ergebnisse
Bei 20 der 70 untersuchten Patienten stellten wir präoperativ ein FTMH < 400 µm und eine VMT < 1500 μm fest. Eine Woche nach intravitrealer Injektion (IVT) mit Ocriplasmin waren 8 der 20 FTMHs bereits verschlossen. Insgesamt zeigten 12 Patienten nach Ocriplasmin IVT einen FTMH-Verschluss. Vier Patienten mit VMT entwickelten nach Ocriplasmin IVT ein neues FTMH. Bei 51 Patienten konnten wir eine Lösung der VMA (vitreomakuläre Adhäsion) feststellen. Drei Patienten entwickelten nach Ocriplasmin Gabe eine rhegmatogene Netzhautablösung. Nach der Behandlung mit Ocriplasmin haben wir bei 8 Patienten Veränderungen in der EZ und ELM festgestellt.
Schlussfolgerungen
Ocriplasmin ist eine minimal-invasive Option bei der Therapie von VMT mit oder ohne kleinen FTMH. Dennoch scheint es einige spezifische Ocriplasmin-assoziierte Risiken zu geben, die meistens nur vorübergehend sind. Schwere Komplikationen wie zum Beispiel Netzhautablösungen sind zwar selten, kommen jedoch vor. Daher sollte jede Indikation für Ocriplasmin sorgfältig geprüft werden.
Abstract
Purpose
Ocriplasmin (Jetrea®) is a therapeutic option for patients with focal vitreomacular traction (VMT) with or without small full thickness macular holes (FTMH) < 400 µm. Retinal alterations after injection with ocriplasmin have been described. The purpose of this essay was to determine Ocriplasmin-associated side-effects and changes in the retinal microstructure.
Methods
We included 70 patients with ocriplasmin treatment in our study. On all patients SD-OCT (spectral-domain optical coherence tomography) scans were performed prior to injection with Ocriplasmin. If present, adverse events were registered. The OCT scans were then evaluated taking the following into account: macular hole (MH) size, macular edema, subretinal fluid (SRF), changes in the ellipsoid zone (EZ) and the external limiting membrane (ELM).
Results
Twenty of the 70 examined patients showed a preoperative FTMH. One week after ocriplasmin IVI (intravitreal injection) 8 of the 20 FTMHs were already closed. Overall 12 patients showed a FTMH closure and 4 patients developed a FTMH after ocriplasmin IVI. Twelve of the 24 MH (macular hole) patients still required an operative closure of the FTMH. We noticed a resolution of the VMT on 51 patients. Three patients developed a retinal detachment. Furthermore, after ocriplasmin IVI we detected changes in the EZ and ELM on 8 patients.
Conclusions
Ocriplasmin is a substantial minimal invasive option in the therapy of VMT with or without small FTMH. Nevertheless, there seem to be some specific ocriplasmin-associated risks, although usually transient. Severe complications like retinal detachment are rare but exist. Therefore, every indication of ocriplasmin should be considered carefully.
