Zusammenfassung
Wir berichten über einen 36-jährigen Patienten, der sich mit einer bereits seit Jahren bestehenden zunehmenden, beidseitigen Visusminderung vorstellte. Anamnestisch waren ein Zustand nach Nierentransplantation sowie eine Innenohrschwerhörigkeit bekannt. Spaltlampenmikroskopisch zeigten sich eine beidseitige, axiale kegelförmige Vorwölbung der Linsenvorderfläche sowie am Hornhautendothel ein leicht vesikuläres Erscheinungsbild. Funduskopisch ließen sich leichte perimakuläre fleckförmige Veränderungen beobachten. In Zusammenschau aller Befunde liegt ein Lentikonus anterior im Rahmen eines Alport-Syndroms vor, bei dem es sich um eine seltene, meist X-chromosomal vererbte Erkrankung handelt. Neben einer Niereninsuffizienz und einer Innenohrschwerhörigkeit, die sich bereits früh manifestieren, treten okuläre Veränderungen erst spät auf. Ein Lentikonus bei Patienten mit einem Alport-Syndrom lässt sich mittels der Phakoemulsifikation mit Implantation einer Hinterkammerlinse sehr gut behandeln. Die Visusprognose nach einer solchen Operation ist exzellent.
Abstract
We present the case of a 36-year-old male patient who presented with an increasing bilateral loss of vision which had existed for several years. Slit-lamp examination revealed a conical anterior protusion of the lens and funduscopy showed a discreet perimacular dot and fleck retinopathy. In consideration of all clinical findings the patient was diagnosed with anterior lenticonus as an ocular manifestation of an Alport’s syndrome which is a rare X-linked disease. Besides renal failure and hearing loss which occur early, ocular changes usually manifest later on. Patients with a anterior lenticonus can be effectively treated with phacoemulsification and intraocular lens implantation. The visual outcome after surgery is excellent.
