Zusammenfassung
Hintergrund
Aufgrund der noch relativ neuen größeren Verbreitung der lamellierenden Keratoplastik (KPL) liegen bisher nur wenige Daten hinsichtlich des Risikos für das Auftreten von Immunreaktionen vor. Ziel der vorliegenden Arbeit war es daher, die Inzidenz von Immunreaktionen bei posteriorer lamellierender KPL zu erfassen und mit der Inzidenz von Immunreaktionen nach perforierender KPL zu vergleichen.
Patienten und Methoden
In diese retrospektive Untersuchung einbezogen wurden die Daten von insgesamt 204 Keratoplastikpatienten der Augenklinik Charité - Universitätsmedizin Berlin, Campus Virchow-Klinikum, die von 1999 bis 2000 sowie von 2007 bis 2009 operiert wurden. Vollständige Daten lagen von 160 Patienten vor. Die Auswertung erfolgte mittels SPSS (Statistical Package for the Social Sciences), Version 12.0. Neben dem Mittelwertvergleich und der rein deskriptiven Auswertung wurde der Chi-Quadrat-Test zur Auswertung eingesetzt.
Ergebnisse
Von den 160 Patienten, die in dieser Studie untersucht wurden, erhielten 29 Patienten eine hintere lamellierende und 131 Patienten eine perforierende KPL. Dabei zeigte sich im Verlauf bei 3,4% der Patienten mit hinterer lamellierender KPL und bei 29,8% der Patienten mit perforierender KPL eine Immunreaktion (χ2 = 9,02; p < 0,001). Von den 44 Patienten mit Fuchs-Endotheldystrophie erhielten 28 Patienten eine hintere lamellierende („Descemet’s stripping with automated endothelial keratoplasty“, DSAEK) und 16 Patienten eine perforierende KPL. Dabei zeigte sich bei 3,6% der Patienten mit DSAEK und bei 18,75% der Patienten mit perforierender KPL eine Immunreaktion.
Schlussfolgerung
Immunreaktionen traten in dieser retrospektiven Studie mit univariater Analyse der Risikofaktoren nach DSAEK erheblich seltener als nach perforierender KPL auf. Der Gesamtvergleich beider Gruppen wurde erheblich durch das stark differente Indikationsspektrum beeinflusst. Auch bei gleicher Indikation (Fuchs-Endotheldystrophie) wies die DSAEK deutlich seltener Immunreaktionen auf.
Abstract
Background
Posterior lamellar keratoplasty is a relatively new surgical procedure for replacing the diseased endothelial layer of the cornea with a healthy layer. There are only few data regarding graft rejection in posterior lamellar keratoplasty. The objective of this paper was to compare posterior lamellar keratoplasty with penetrating keratoplasty for the incidence and risk of graft rejection.
Methods
This retrospective study included a total of 204 consecutive patients who underwent penetrating keratoplasty or posterior lamellar keratoplasty at the Department of Ophthalmology, Charité CVK, between 1999 and 2000 and between 2007 and 2009. Complete data were obtained in 160 cases. Statistical analysis was performed using SPSS (Statistical Package for the Social Sciences), version 12.0. Descriptive statistics, χ2-test and mean values were used to describe and assess the data.
Results
Out of 160 eyes 29 were treated with posterior lamellar keratoplasty and 131 eyes were treated with penetrating keratoplasty. The incidence of graft rejection was 3.4% in the posterior lamellar group and 29.8% in the penetrating keratoplasty group (χ2 = 9.02, p < 0.001). Of 44 patients with Fuchs’ endothelial dystrophy (FED) 28 underwent posterior lamellar keratoplasty and 16 patients were treated with penetrating keratoplasty. The incidence of graft rejection in this group was 3.6% after posterior lamellar keratoplasty and 18.75% after penetrating keratoplasty.
Conclusions
In this retrospective study with univariate analysis of risk factors, graft rejection was less frequent after posterior lamellar keratoplasty than after penetrating keratoplasty. A comparison of both groups was limited because of different indications for surgery. Posterior lamellar keratoplasty decreased the risk of graft rejection under conditions with the same indications (Fuchs’ endothelial dystrophy).
